Diabetic foot ulcers are urgent. 75-85% heal with aggressive treatment in 12-20 weeks. Treatment: debridement, off-loading (boot, total contact cast), wound dressings, infection management, vascular evaluation, glucose optimization. Untreated ulcers lead to amputation.
Treatment Hierarchy
1. Debridement (remove devitalized tissue).
2. Off-loading (boot, total contact cast, custom orthotic with cutout).
3. Wound dressings (alginate, hydrogel, foam).
4. Antibiotics if infected.
5. Cellular/tissue products (Apligraf, Dermagraft) for refractory.
6. Vascular intervention if PAD.
7. Hyperbaric oxygen for select cases.
