While often dismissed as “minor” wounds due to the thin overlying skin and superficial tendon location, untreated extensor tendon injuries produce functional deficit, secondary contracture, and footwear limitations.
Extensor Tendon Anatomy
The extensor tendons of the foot include: extensor hallucis longus (EHL), which extends the hallux and assists ankle dorsiflexion; extensor digitorum longus (EDL), with four slips extending the lesser toes; extensor digitorum brevis (EDB) and extensor hallucis brevis (EHB), the intrinsic extensors arising from the calcaneus. The extensor retinaculum at the ankle maintains tendon position during dorsiflexion. Dorsal foot lacerations typically involve multiple tendons simultaneously — all injured tendons must be identified and repaired individually for full restoration of function.

Primary repair uses non-absorbable suture in a modified Kessler or figure-of-eight configuration with epitendinous running suture to smooth the repair site. The ankle is immobilized in mild dorsiflexion for 4 weeks post-repair to reduce repair tension, followed by progressive range of motion.
