Nighttime foot pain that disrupts sleep is one of the most quality-of-life-affecting complaints in podiatric medicine. Unlike daytime foot pain that can be managed with rest, nighttime pain strikes when you’re trying to recover — making it particularly distressing. The cause determines the treatment, and several conditions specifically worsen at night.

Top Causes of Nighttime Foot Pain

1. Peripheral Neuropathy

By far the most common cause of foot pain that is specifically worse at night. Peripheral neuropathy — most commonly from diabetes but also from vitamin B12 deficiency, alcohol use, or chemotherapy — causes burning, tingling, and electric-shock sensations that intensify at rest. Without daytime distractions and activity dampening the symptoms, nighttime neuropathy pain becomes unbearable. Treatment: blood glucose optimization, gabapentin, pregabalin, duloxetine, topical treatments, and emerging MLS laser therapy.

2. Plantar Fasciitis (Morning Pain Variant)

While plantar fasciitis typically causes morning pain with first steps, severe cases can cause throbbing heel pain at night as the fascia tightens during extended rest. Night splints that hold the foot in a dorsiflexed position (stretching the fascia overnight) dramatically reduce both nighttime and morning pain. This is one of the most underutilized and highly effective treatments for plantar fasciitis.

3. Restless Legs Syndrome (RLS)

RLS causes an irresistible urge to move the legs, usually accompanied by uncomfortable crawling, itching, or aching sensations deep in the legs and feet. It is specifically worse at rest and at night, and is temporarily relieved by movement. RLS has a neurological rather than structural cause and is treated with dopamine agonists and addressing iron deficiency.

4. Gout

Gout attacks famously strike at night — typically in the big toe joint. Uric acid crystals precipitate more readily at lower body temperatures, and the big toe (farthest from the heart, coolest in the body) is the most common site. A gout flare causes intense, excruciating pain, redness, and swelling that makes even the weight of a bedsheet unbearable. Treatment: colchicine or indomethacin for acute flares, long-term urate-lowering therapy (allopurinol).

5. Arthritis

Both osteoarthritis and inflammatory arthritis (rheumatoid, psoriatic) can cause significant nighttime joint pain. Inflammatory arthritis characteristically causes morning stiffness lasting more than one hour alongside nighttime pain. X-ray and blood work helps differentiate the type and guides treatment.

6. Cramps and Spasms

Nighttime foot and toe cramps are extremely common, particularly in athletes and the elderly. They are typically caused by electrolyte imbalance (magnesium, potassium, sodium), dehydration, nerve irritation, or circulatory changes at rest. Quinine was historically used but is no longer recommended due to side effects; magnesium glycinate supplementation and adequate hydration are first-line approaches.

7. Peripheral Artery Disease

In severe PAD, rest pain develops when the blood supply is insufficient to meet even resting metabolic demands. Characteristically, this pain worsens when lying flat (gravity no longer assists flow) and is relieved when hanging the foot off the bed or standing. Rest pain is a late sign of PAD and requires urgent vascular evaluation.
