Why Nighttime Foot Pain Has Different Causes Than Daytime Pain
Foot pain that improves with rest (stops when you sit or lie down) is typical of mechanical overuse conditions — plantar fasciitis, metatarsalgia, stress fractures. Foot pain that is present or worsens at night despite no weight-bearing is a different clinical pattern and suggests non-mechanical causes: nerve pathology, vascular insufficiency, or systemic inflammatory conditions.
The Most Common Causes of Nocturnal Foot Pain
Peripheral neuropathy: The most common cause of nighttime foot pain in our practice. Burning, tingling, electric, or aching sensations in the feet at rest are classic symptoms of diabetic peripheral neuropathy, idiopathic small fiber neuropathy, or other neuropathic conditions. The pain is often worse at night because there are fewer competing sensory inputs (no visual input, less ambient sensation) and because lying flat alters lower extremity circulation.
Peripheral arterial disease (PAD): Rest pain (ischemic rest pain) in the forefoot or toes at night is a serious symptom — it indicates arterial insufficiency severe enough that the foot cannot receive adequate blood flow even without the increased demand of walking. This warrants urgent vascular evaluation. PAD rest pain is typically relieved by hanging the leg off the bed (gravity improves perfusion).
Tarsal tunnel syndrome: Compression of the tibial nerve at the medial ankle can produce burning, tingling, and numbness in the sole and toes, often worse at rest or at night. Distinguished from peripheral neuropathy by typically unilateral symptoms and a positive Tinel’s sign at the tarsal tunnel.
Inflammatory arthritis: Rheumatoid arthritis, psoriatic arthritis, and gout can all cause nocturnal foot pain. The inflammatory cytokine cycle has a diurnal pattern — symptoms are often worst in the early morning hours (2–6 AM).

Is foot pain at night serious? It depends on the cause, but nocturnal foot pain warrants evaluation more urgently than daytime pain from overuse. Burning/tingling at rest suggests neuropathy; rest pain with relief when dangling the leg suggests vascular disease. Both require clinical diagnosis.
Can plantar fasciitis cause night pain? Typically no — plantar fasciitis is a mechanical condition that improves with rest. Persistent foot pain at night in someone with “plantar fasciitis” should prompt reassessment for nerve entrapment, neuropathy, or a stress fracture.
