How to Treat an Ingrown Toenail at Home (And When to See a Doctor)
Ingrown toenails are painful, common, and very treatable. The good news: mild cases can often be managed at home with proper technique. The bad news: most people use the wrong techniques — digging, cutting corners, and creating a cycle of worsening ingrowns. Here’s the right approach, and the clear line between “home treatment is fine” and “you need to see us today.”
What Causes Ingrown Toenails
An ingrown toenail develops when the nail edge grows into or presses against the surrounding skin (the nail fold). Primary causes: cutting nails too short or rounding the corners (the #1 cause), narrow-toed shoes compressing the nail fold, genetic nail curvature, toe injuries, and nail conditions like fungus that alter nail shape.
The Stages: Which Stage Are You In?
Stage 1 (Mild): Redness, slight tenderness, no infection. Safe to treat at home.
Stage 2 (Moderate): Increased pain, swelling, possible fluid. May be able to treat at home with care.
Stage 3 (Severe): Significant swelling, pus or discharge, granulation tissue (proud flesh), warmth. See a podiatrist today — this is infected.
Home Treatment for Stage 1-2
- Warm soak: 15-20 minutes in warm water 2-3 times daily. Softens the skin and nail, reduces inflammation, and often allows the nail to grow away from the skin naturally.
- Gentle lifting: After soaking, use a clean, rounded toothpick or dental floss to gently lift the nail edge away from the skin. Place a tiny piece of cotton or dental floss under the nail edge to keep it lifted. Do NOT dig or force.
- Topical antiseptic: Apply antibiotic ointment to prevent infection after each soak.
- Proper footwear: Wide-toed shoes only until the ingrown resolves. Avoid tight socks.
- Correct future nail cutting: Cut straight across, never round corners, leave 1-2mm beyond the toe tip.
When to See a Podiatrist Immediately
Do not attempt home treatment if: there is pus or discharge (infected), you have diabetes or neuropathy (any foot infection is an emergency), the pain is severe, redness is spreading up the toe, you’ve tried home treatment for 1 week with no improvement, or this is a recurrent ingrown toenail (structural correction may be needed).
In-office treatment for an ingrown toenail is quick and essentially painless after local anesthetic — a partial nail avulsion (removing the offending nail edge) takes about 15 minutes and provides immediate relief.

How long does a mild ingrown toenail take to heal at home?
With consistent soaking and proper care, mild Stage 1 ingrowns typically resolve in 1-2 weeks. If not improving in 5-7 days, see a podiatrist.
What’s the difference between ingrown toenail removal and the permanent procedure?
A standard partial nail avulsion removes the offending nail edge — it grows back (usually with a properly curved shape) in 3-6 months. A permanent procedure (matrixectomy) uses phenol to destroy the nail-forming cells on one side, so that section of nail never grows back. Recommended for recurrent ingrown toenails — very effective long-term solution.
Is ingrown toenail treatment painful?
The injection of local anesthetic causes brief stinging (30-60 seconds). After that, the area is fully numb and the procedure itself is painless. Most patients are surprised how comfortable it is.

For ingrown toenails that don’t respond to home treatment — or if you notice signs of infection (pus, spreading redness, warmth) — our clinic offers same-day nail procedures under local anesthesia. Dr. Tom performs partial nail avulsion with chemical matrixectomy to permanently prevent recurrence on the affected nail border. The procedure takes about 15 minutes and most patients return to normal shoes within 2-3 days.

Doctor Hoy’s Natural Pain Relief Gel — Natural topical pain relief I use in our clinic. Apply around (not directly on) the inflamed nail border for comfort between soaks. Not ideal for: open infected wounds — see a podiatrist instead.
PowerStep Pinnacle Orthotic Insoles — Proper arch support reduces toe pressure from shoe compression, which contributes to recurrent ingrown nails. Not ideal for: acute infection (address the ingrown first).

Frequently Asked Questions About Ingrown Toenail Home Treatment
Can I fix an ingrown toenail at home?
Mild ingrown toenails (slight redness and tenderness without pus) can often improve with warm epsom salt soaks 2-3x daily for 15 minutes, followed by gently placing a small piece of cotton or dental floss under the nail edge. Cut nails straight across — never round the corners. If symptoms worsen or don’t improve within 5-7 days, see a podiatrist.
When should I see a doctor for an ingrown toenail?
See a podiatrist same-day if you notice pus or drainage, red streaking up the toe, fever, increasing pain despite home treatment, or if you have diabetes or poor circulation. Diabetic patients should never attempt home treatment for ingrown toenails — the infection risk is too high.
How do I prevent ingrown toenails from coming back?
Cut toenails straight across (never curve the corners), wear shoes with adequate toe box width, keep nails at moderate length (not too short), and wear moisture-wicking socks. If you get recurrent ingrown nails on the same toe, a permanent nail border removal procedure can prevent regrowth on that side.
