Baxter Nerve Entrapment: The Hidden Cause of Medial Heel Pain
The first branch of the lateral plantar nerve (Baxter nerve) is entrapped between the deep fascia of the abductor hallucis muscle and the medial head of the quadratus plantae (QP) as it courses horizontally across the medial heel to reach the abductor digiti quinti muscle. Compression at this point causes medial heel burning and aching that is often attributed entirely to plantar fasciitis — and indeed, the two conditions frequently coexist, because the same overpronation mechanics that stress the plantar fascia also compress the Baxter nerve in the tight medial heel compartment. The distinguishing features are a burning or electric quality to the pain (neuropathic component), constant aching in addition to the mechanical first-step pattern, and a positive Tinel sign directly on the medial heel (not the anteromedial tuberosity where plantar fasciitis is maximally tender). Baxter nerve entrapment is estimated to coexist in 15–20% of plantar fasciitis cases and explains many “failed plantar fasciitis treatment” patients.
Treatment of Medial Heel Pain
Most medial heel pain from plantar fasciitis responds to stretching, orthotics, and injections with the same protocol used for inferior heel pain. When a neuropathic component suggests Baxter nerve entrapment, corticosteroid injection specifically targeting the medial heel at the Baxter nerve entrapment site (between abductor hallucis and QP) is both diagnostic and therapeutic — significant relief after injection confirms the diagnosis. Orthotics that address overpronation reduce both fascial and neural compression simultaneously. When conservative care fails after 3–6 months and nerve conduction studies or MRI support the diagnosis, surgical decompression of the Baxter nerve — performed through a medial incision releasing the deep abductor hallucis fascia — provides 80–85% relief in appropriately selected patients. Tarsal tunnel syndrome requires its own specific surgical decompression at the posteromedial ankle if non-surgical treatment fails.
