In the foot, it most commonly develops beneath a diabetic foot ulcer that has been present for weeks, silently infecting the bone below. Left untreated, it can spread to adjacent joints and soft tissue, ultimately requiring amputation. Early diagnosis and targeted antibiotic therapy are the difference between bone salvage and limb loss.
What Is Osteomyelitis?
Osteomyelitis (OSS-tee-oh-my-uh-LY-tis) is an infection of bone and bone marrow. In the foot, it occurs via two main routes:
- Contiguous spread (most common in the foot): Infection spreads from an overlying soft tissue wound, ulcer, or infected joint directly into bone. This is the pathway in diabetic foot osteomyelitis — the bone beneath a foot ulcer becomes infected from the wound.
- Hematogenous spread: Bacteria seed bone through the bloodstream from a distant source (e.g., skin infection, dental procedure, IV line). More common in children and immunocompromised patients.
- Direct inoculation: Puncture wound, surgery, or open fracture introduces bacteria directly into bone.
The most common causative organism is Staphylococcus aureus (including MRSA), accounting for 70–80% of cases. Diabetic foot osteomyelitis often involves polymicrobial infections including gram-negative rods and anaerobes from the wound bed.
Who Gets Osteomyelitis in the Foot?
Risk factors for foot osteomyelitis include:
- Diabetes with peripheral neuropathy: Loss of protective sensation means wounds go unnoticed until infected; impaired immune response delays bacterial clearance
- Peripheral arterial disease (PAD): Reduced blood flow impairs antibiotic delivery and immune cell trafficking to infected tissue
- Chronic foot ulcers: Any non-healing wound present > 4 weeks should be evaluated for underlying osteomyelitis

Laboratory Tests
- ESR (erythrocyte sedimentation rate): Elevated in >90% of osteomyelitis cases. A useful screening marker.
- CRP (C-reactive protein): More sensitive and faster to respond than ESR; excellent for monitoring treatment response
- WBC (white blood cell count): Often normal in chronic osteomyelitis — do not use as the sole screening tool
- Blood cultures: Essential before starting antibiotics in hematogenous infection; often negative in contiguous osteomyelitis
Imaging
- X-ray: First step but low sensitivity. Cortical destruction (the definitive sign) doesn’t appear until 30–50% of bone is destroyed, which takes 10–21 days. Early infection may be completely invisible.
- MRI: Gold standard — sensitivity 90%, specificity 83%. Shows bone marrow edema (T2 hyperintensity) and cortical destruction within days of infection onset. Also maps extent of soft tissue involvement for surgical planning.
- White cell scan (111-In leukocyte scan): Used when MRI is contraindicated (hardware artifact). Also useful to distinguish Charcot neuroarthropathy from osteomyelitis — a common clinical dilemma.
- CT scan: Best for defining cortical destruction in chronic cases and guiding surgical debridement.
Bone Biopsy — The Gold Standard
Percutaneous or surgical bone biopsy with culture is the definitive diagnosis and, critically, provides pathogen identification for targeted antibiotic therapy. Superficial wound swabs do not reliably predict the organism causing osteomyelitis — bone culture results differ from swab cultures in 50–60% of cases. Antibiotic Therapy
Antibiotic selection is based on bone biopsy culture and sensitivity results. Standard treatment duration is 4–6 weeks:
- MSSA (methicillin-sensitive Staph aureus): IV nafcillin/oxacillin or oral dicloxacillin; highly bioavailable oral alternatives: rifampin + fluoroquinolone for chronic infection
- MRSA: IV vancomycin (first-line); oral linezolid, trimethoprim-sulfamethoxazole, or doxycycline for step-down therapy
- Pseudomonas aeruginosa (sneaker puncture): Anti-pseudomonal beta-lactam (piperacillin-tazobactam) or fluoroquinolone
- Polymicrobial diabetic foot: Broad-spectrum coverage including gram-negatives and anaerobes (ertapenem, piperacillin-tazobactam, or carbapenem for severe cases)
The IDSA 2012 guidelines allow oral antibiotics with high bioavailability (fluoroquinolones, linezolid, trimethoprim-sulfamethoxazole) as definitive therapy in selected cases, avoiding PICC line complications. We coordinate with infectious disease specialists for complex cases.
Surgical Treatment
- Sequestrectomy: Removal of necrotic bone (sequestrum) — essential in chronic osteomyelitis where avascular bone cannot be sterilized with antibiotics alone

- Minor amputation + wound closure: Resection of infected bone to viable margins with primary or flap closure
- Revascularization: When PAD limits antibiotic delivery, vascular surgery consultation for bypass or angioplasty improves cure rates
Recovery and Prognosis
Acute osteomyelitis in healthy patients treated promptly responds well — cure rates exceed 90% with appropriate antibiotics. Chronic diabetic foot osteomyelitis is far more challenging: recurrence rates reach 30% at 1 year even with optimal treatment. Key prognostic factors include adequacy of blood supply, ability to offload the wound, compliance with the full antibiotic course, and glycemic control.
ESR and CRP are monitored every 2–4 weeks during treatment. Failure of CRP to normalize after 4 weeks of appropriate antibiotics suggests inadequate surgical debridement, antibiotic resistance, or an undetected abscess requiring imaging re-evaluation.

- Foot ulcer with red streaking (lymphangitis) or rapidly spreading redness
