Pre-operative optimization — systematic modification of modifiable risk factors before elective foot and ankle surgery — is one of the highest-use interventions available to improve surgical outcomes. Three factors dominate the pre-operative optimization evidence base: nutritional status, cigarette smoking, and glycemic control. Each independently predicts wound complication, infection, non-union, and implant failure rates; optimizing all three before surgery produces additive risk reduction.

Nutritional Status

Malnutrition significantly impairs wound healing, immune function, and bone healing. Laboratory markers: serum albumin <3.5 g/dL, prealbumin <15 mg/dL, and total lymphocyte count <1500/mm³ each independently predict wound complications and non-union. In practice, these markers are most important in elderly patients, patients with inflammatory bowel disease, and patients with diabetes. Pre-operative nutritional intervention (high-protein diet, protein supplementation, vitamin D optimization) for 4–6 weeks before elective surgery in malnourished patients reduces wound complication rates. Zinc deficiency — particularly in elderly patients — impairs the collagen cross-linking required for tendon and bone healing; supplementation is appropriate when deficiency is identified.

Smoking Cessation

Cigarette smoking is one of the most powerful predictors of surgical complications in foot and ankle surgery. Nicotine produces vasoconstriction that reduces wound microcirculation by up to 30–40%; carbon monoxide reduces oxygen-carrying capacity; and smoking impairs collagen synthesis, fibroblast proliferation, and bone healing. The evidence for smoking cessation before surgery is compelling: studies demonstrate that wound complication rates in smokers are 2–4 times higher than in non-smokers for foot and ankle procedures requiring significant dissection or bone work. Cessation for 4–8 weeks before surgery significantly reduces these risks — while cessation for <2 weeks may paradoxically increase wound complications from increased secretions. Nicotine replacement therapy carries similar microvascular risks as cigarettes during the perioperative period.

Glycemic Control

HbA1c as a pre-operative marker is well-validated: HbA1c >8% predicts significantly elevated wound infection, non-union, and implant failure rates. The traditional threshold of HbA1c <8% (or ideally <7%) before elective foot and ankle surgery is supported by evidence from both retrospective and prospective series. The practical challenge is that glycemic optimization can take 3–6 months — surgery should be deferred when possible to allow optimization. For urgent surgery, perioperative glucose targeting of 140–180 mg/dL reduces SSI rates compared to permissive hyperglycemia. Key Nutrients for Foot Health

Vitamin D (25-OH)
