Platelet-rich plasma (PRP) — an autologous blood concentrate containing supraphysiological concentrations of platelets and their associated growth factors — has emerged as a regenerative injection therapy for chronic musculoskeletal conditions of the foot and ankle that have not responded adequately to cortisone injections and physical therapy. Unlike corticosteroids (which reduce inflammation but inhibit tissue repair), PRP delivers growth factors that actively stimulate tendon, ligament, and fascial healing in chronically degenerated tissue.

PRP Preparation and Growth Factor Content

Preparation: 30–60mL of whole blood is drawn from the patient’s peripheral vein and centrifuged at specific speeds to separate blood components — red cells (bottom layer), platelet-poor plasma (top layer), and PRP concentrate (buffy coat layer). PRP contains 3–10× the normal platelet concentration of whole blood. Key growth factors in PRP: PDGF (platelet-derived growth factor) — stimulates tenocyte and fibroblast proliferation; TGF-β (transforming growth factor) — regulates collagen synthesis; VEGF (vascular endothelial growth factor) — promotes neovascularization; EGF (epidermal growth factor) — stimulates cell migration and proliferation. Leukocyte-rich vs. leukocyte-poor PRP: controversial — for tendinopathy, leukocyte-poor PRP may produce better outcomes; for wound healing, leukocyte-rich PRP is preferred.

Indications and Evidence

Plantar fasciitis: Level 1 evidence (multiple RCTs) demonstrates PRP superiority over cortisone injection at 3–6 months for chronic plantar fasciitis — PRP produces more durable pain relief while cortisone produces faster initial improvement but higher recurrence; PRP is particularly appropriate for patients who have failed cortisone injection. Achilles tendinopathy: moderate evidence supports PRP for chronic mid-portion Achilles tendinopathy — PRP combined with eccentric exercise produces better outcomes than eccentric exercise alone. Chronic ankle sprain and ligament injuries: emerging evidence for PRP augmentation of lateral ankle ligament repair in high-risk patients. Patient selection: PRP is most appropriate for conditions of chronic tendinopathy or fasciopathy with degenerated (not inflammatory) tissue — duration >3 months, failed conservative treatment including stretching, orthotics, and cortisone injection.
