Tingling or “pins and needles” in the toes or feet — often described as a feeling of feet “falling asleep” that doesn’t fully resolve. This is frequently the first symptom. Burning sensation in the feet, especially at night — worsened by warmth and improved by cold. Gradual loss of protective sensation — cuts, blisters, and pressure sores that you don’t feel. Changes in foot shape — intrinsic muscle atrophy causing toe curling (hammer toes), foot collapse. Skin changes — excessively dry skin, reduced sweating. Balance problems — difficulty walking on uneven surfaces or in the dark.
