Subungual Hematoma: Drainage Technique and Nail Preservation

A subungual hematoma — blood pooled between the nail plate and nail bed following blunt trauma to the toenail — is one of the most painful acute nail injuries, producing intense throbbing pressure under the nail that can be immediately relieved by trephination (drainage). Understanding the indications for nail removal vs. simple drainage, the technique for pain-free trephination, and the expected healing course prevents unnecessary procedures and unnecessary pain.

Assessment and Drainage Indications

Clinical assessment: the subungual hematoma appears as a dark red, maroon, or black discoloration beneath the nail plate; the extent of discoloration as a percentage of the nail plate area helps guide management. Small hematomas (<25% of nail plate area): typically self-resolving; drainage may not be necessary unless pain is significant. Hematomas >25–50% of nail plate area with pain: drainage (trephination) provides immediate pain relief; is appropriate for any painful subungual hematoma regardless of size. Nail removal indications: when the nail plate is unstable (avulsed partially from the nail bed) or when the nail bed laceration requires suture repair — removal of the nail plate to repair the nail bed under it is required to prevent nail plate dystrophy from an unrepaired nail bed tear. X-ray: always obtained to identify underlying distal phalanx fracture — a distal phalanx fracture beneath a subungual hematoma constitutes an open fracture (the nail bed laceration communicates with the bone), requiring irrigation and appropriate antibiotic coverage.

Trephination Technique and Healing

Trephination technique: no anesthesia is required — the procedure is painless when performed through the nail plate because the nail plate has no sensory innervation; the nail plate is trephinated using a heated paper clip (traditional), electrocautery device, or 18-gauge needle rotated between fingers over the hematoma; a small hole allows immediate drainage of the accumulated blood; pressure under the nail relieves immediately. Multiple punctures may be needed for large hematomas. Post-procedure: soak daily, antibiotic ointment; watch for signs of infection (paronychia). Nail healing: the nail plate remains in place and the nail bed heals beneath it; the nail grows forward over 3–4 months; some nail discoloration or ridging may persist.
