Tennis involves rapid lateral movement, sudden stops and direction changes, explosive push-off during serves and groundstrokes, and prolonged play on hard courts. These mechanics create predictable injury patterns concentrated in the foot, ankle, and lower leg. Understanding them — and the footwear choices that modify risk — helps players of all levels extend their playing careers.
Most Common Tennis Foot Injuries
Ankle sprains occur during lateral movement and court coverage, particularly when the foot is inverted during rapid direction change. The lateral ankle complex — ATFL and CFL — is most vulnerable. Proper ankle rehabilitation after the first sprain is critical to prevent the chronic instability that leads to repeated sprains and eventual ankle arthritis. Plantar fasciitis is prevalent in tennis players due to the explosive push-off demands of the serve and the prolonged standing and quick direction changes on hard courts. Players with tight calves and high arches are particularly susceptible.
