TAA now represents a legitimate alternative to ankle arthrodesis (fusion) for end-stage ankle osteoarthritis in appropriately selected patients, offering preservation of tibiotalar motion and potentially better function for patients who wish to remain active.

Indications, Contraindications, and Patient Selection

Primary indication: end-stage ankle osteoarthritis (post-traumatic — the most common indication; primary osteoarthritis; inflammatory arthritis — rheumatoid arthritis) with adequate bone stock, near-normal ankle alignment, and failure of conservative management. Ideal TAA candidate: age 55–70 (lower activity demands than younger patients; adequate bone maturity vs. older fragile bone); BMI <35; near-neutral coronal plane alignment (<10° varus/valgus); intact ligamentous stability; adequate bone stock; no avascular necrosis of the talus. Contraindications: avascular necrosis of the talus (poor bone support for the talar component); severe osteoporosis; uncontrolled diabetes or peripheral neuropathy (Charcot risk); active infection; severe malalignment (>15–20° varus/valgus — requires concurrent osteotomy or arthrodesis). TAA vs. ankle arthrodesis: arthrodesis produces reliable, durable pain relief with 90–95% fusion rate; TAA preserves tibiotalar motion (15–20° arc) but may accelerate subtalar and midtarsal joint degeneration from altered biomechanics; both achieve similar patient-reported outcomes in appropriately selected patients.

Modern Implants and Outcomes

Third-generation implants: INBONE (fully constrained, intramedullary stem fixation), STAR (Scandinavian Total Ankle Replacement — mobile bearing, 3-component), Salto-Talaris, INFINITY — all use cementless osseointegration with porous metal coating. Outcomes: 85–90% implant survivorship at 10 years with third-generation designs; AOFAS score improvement of 30–40 points; meaningful improvement in gait velocity and single-limb stance time. Revision: failed TAA can be revised to another TAA (if adequate bone stock remains) or converted to arthrodesis — conversion arthrodesis with bone graft for large bone defects achieves 85–90% union rates.
