Turf toe is a sprain of the plantar capsuloligamentous complex of the first metatarsophalangeal (MTP) joint — the big toe joint — caused by hyperextension. It is a common sports injury in football, soccer, and basketball players. What Causes Turf Toe?

Turf toe occurs when the great toe is forced into excessive dorsiflexion (upward bending) while the forefoot is planted on the ground — the classic mechanism of pushing off while falling forward. The rigid surface of artificial turf combined with flexible, low-profile athletic shoes is the prototypical scenario. The injury can also occur on natural grass and in non-athletes from a fall or collision.

Turf Toe Grades

• Grade 1: Stretching of the plantar capsule. Mild pain and swelling; minimal tenderness.
• Grade 2: Partial tear. Moderate pain, swelling, and bruising; restricted range of motion.
• Grade 3: Complete tear of the capsuloligamentous complex. Severe pain, significant swelling and ecchymosis, inability to push off, possible sesamoid fracture or dislocation.

Symptoms

• Immediate pain at the ball of the foot at the base of the big toe
• Swelling and bruising under and around the first MTP joint
• Pain with toe pushoff or dorsiflexion
• Limited big toe upward motion
• Instability of the first MTP joint (Grade 3)

Diagnosis

Dr. Biernacki evaluates range of motion, joint stability, sesamoid position, and the integrity of the plantar plate. Weight-bearing X-rays assess sesamoid fracture or proximal migration. MRI is ordered for Grade 2–3 injuries to quantify ligament and plantar plate damage and determine whether surgical repair is warranted.

Treatment

Grade 1: RICE protocol (rest, ice, compression, elevation), buddy taping, and a stiff-soled shoe for 1–2 weeks. Athletes can often return to play with taping in 3–5 days.

Grade 2: CAM boot or rigid-soled shoe for 1–3 weeks with protected weight-bearing. Physical therapy targeting range of motion and strength. Return to sport at 2–6 weeks with taping and a rigid carbon-fiber insole.

Grade 3: Non-weight-bearing cast or boot for 3–6 weeks, followed by progressive rehabilitation. Surgery is indicated for complete plantar plate disruption, irreducible sesamoid fracture or diastasis, vertical instability of the first MTP joint, or loose body in the joint. Dr. Biernacki performs repair of the capsuloligamentous complex, sesamoid fixation, or sesamoidectomy as appropriate.
