Quick answer: Verrucas (plantar warts) on the feet are treated with salicylic acid, cryotherapy (liquid nitrogen), Swift microwave therapy, or in-office procedures. Most verrucas resolve with persistent treatment over 6-12 weeks. Swift microwave therapy has the highest single-session success rate at 70-80% resolution in 3 sessions. Untreated verrucas can persist for years and spread to other areas.

How do I know if I have a verruca or a callus?

The pinch test is the most reliable way to tell them apart at home: press the lesion from the sides (like squeezing it). A verruca is more painful with side-to-side pressure. A callus hurts more with direct downward pressure. Verrucas also have tiny black dots (thrombosed capillaries), which look like seeds — calluses don’t have these. When you pare down the dead skin, a verruca will reveal a smooth vascular core with capillary bleeding; a callus reveals uniform yellow-white tissue without bleeding. If uncertain, a podiatrist can confirm with clinical examination in seconds.

Can verrucas go away on their own?

Yes — approximately 65% of verrucas in children resolve spontaneously within 2 years; adults have lower spontaneous resolution rates (around 30-40% over 2 years). However, waiting means the wart can grow larger, spread to adjacent areas, and become harder to treat. Children who are asymptomatic can be observed; adults and anyone with a symptomatic verruca on a weight-bearing area should treat promptly rather than wait.

How many Swift sessions are needed to clear a verruca?

The standard Swift protocol is 3 sessions spaced 4 weeks apart. Most patients see significant improvement after the first or second session. Published studies show 70-80% of patients achieve complete clearance within the 3-session protocol. For mosaic warts (multiple clustered verrucas), a 4th session may be recommended. Swift is more effective than cryotherapy for larger, multiple, or previously treatment-resistant verrucas.

Bottom line: Start with 40% salicylic acid consistently for 6-8 weeks. If not improving, professional cryotherapy or Swift microwave therapy offers significantly higher success rates. Swift is the best option for multiple, large, or recalcitrant verrucas — 70-80% clearance in 3 sessions. Don’t let a verruca persist for years; effective treatment is available.

Salicylic Acid — First-Line Home Treatment

Salicylic acid (SA) in concentrations of 17-40% is the standard first-line treatment for verrucas. Available OTC as liquids (Compound W, Wart Remover), pads (Dr. Scholl’s), and gels. Higher concentrations (40%) are more effective but require careful application to avoid damaging surrounding normal skin.

Protocol: Soak the foot in warm water for 5 minutes to soften the skin. Use an emery board or pumice stone to gently file down the dead white skin on the wart surface. Apply SA directly to the wart only (use a toothpick or cotton swab for precision). Let dry completely. Cover with adhesive tape overnight. Repeat every 24-48 hours for 6-12 weeks. Success rate with consistent application: approximately 50-75% over 3 months.

Cryotherapy (Liquid Nitrogen) — In-Office Treatment

Cryotherapy freezes the wart tissue with liquid nitrogen at -196°C, destroying the HPV-infected cells. In-office cryotherapy is more aggressive than OTC freeze sprays (which only reach around -57°C) and more effective. Treatments are done every 2-4 weeks; multiple sessions are typically required.

After freezing, a blister forms and the wart tissue sloughs off over 1-2 weeks. The area needs to be kept clean and covered. Cryotherapy success rates: approximately 60-70% over 3-6 sessions. It can be painful, particularly on the plantar surface. We use topical anesthetic to improve tolerance.

Swift Microwave Therapy — Best Single-Modality Success Rate

Swift is a relatively new treatment that uses targeted microwave energy to heat water molecules within the wart tissue, triggering an immune response against the HPV virus. Unlike cryotherapy, which destroys tissue from outside in, Swift works by activating the patient’s own immune system to fight HPV throughout the body — which explains why it can clear multiple warts simultaneously and prevent recurrence more effectively than destructive methods.

Swift is delivered in 3 sessions spaced 4 weeks apart. Success rate: approximately 70-80% clearance across published studies. It’s fast (seconds per wart), no anesthetic required, no open wound, no dressing needed. It costs more than cryotherapy but the faster resolution and lower recurrence make it cost-effective for stubborn or multiple verrucas. We offer Swift therapy at both our Howell and Bloomfield Township offices.

Other Treatment Options

Duct tape occlusion — Covering the wart with duct tape for 6 days, removing, filing, and repeating. One randomized controlled trial showed superiority over cryotherapy; other studies were less positive. It’s painless and costs nothing — worth trying alongside other treatments. Bleomycin injection — Chemotherapy agent injected into the wart; very effective (80-90% clearance) for recalcitrant warts but reserved for cases that have failed multiple other treatments due to cost and procedural complexity. Candida antigen injection — Stimulates a local immune response; effective for multiple warts simultaneously. Laser (CO2 or pulsed dye) — Vaporizes wart tissue; effective but more expensive and may leave scarring. Surgical excision — Reserved for very large or recalcitrant warts; higher scarring risk and the scar can itself cause pain with weight bearing.

⚠️ See a podiatrist for your verruca if:

  • You’ve tried OTC SA for 3 months without improvement
  • The wart is large (over 1cm diameter) or you have multiple warts
  • It’s on a weight-bearing area and causing significant pain
  • You have diabetes, compromised immunity, or poor circulation
  • You’re unsure if it’s actually a wart (could be corn, callus, porokeratosis, or rarely amelanotic melanoma)

Preventing Verrucas from Spreading

HPV spreads through direct skin contact with contaminated surfaces in warm, moist environments — swimming pools, gym locker rooms, and communal showers are the classic exposure sites. Prevention: wear flip flops in communal shower areas, don’t share towels, keep feet clean and dry, don’t pick at verrucas (this spreads HPV to surrounding skin and fingers), cover active warts with waterproof tape before swimming. If you’re treating a verruca, wash hands thoroughly after touching the area.

In-Office Treatment at Balance Foot & Ankle

We offer cryotherapy and Swift microwave therapy for plantar warts and verrucas. If you’ve been struggling with a persistent wart, professional treatment is much more effective than continued home attempts. Swift therapy in particular often resolves verrucas in 3 sessions that have resisted years of home treatment.
