These demands create injury patterns not seen in other sports. Conservative management includes rest modification, physical therapy, and corticosteroid injection at the tendon sheath.

Gymnasts who train barefoot or in thin slippers absorb forefoot impact directly through the sesamoid bones beneath the first metatarsal head. Sesamoiditis, sesamoid stress fractures, and occasionally bipartite sesamoid fragmentation develop from repetitive loading. Offloading orthotics and modification of floor work allow continued training in many cases while healing progresses.

Plantar Fasciitis in Young Athletes

High-arched foot types common in gymnasts — selected in part because of their aesthetic appeal — create increased plantar fascia tension. Plantar heel pain in a competitive gymnast requires careful evaluation to distinguish fasciitis from calcaneal stress fracture, which can occur in this population. Gymnasts training on spring floors and balance beams are largely unshod, making intrinsic foot strength and arch mechanics especially important.
