Collagen: The Body’s Structural Protein
Collagen is the most abundant protein in the body, constituting approximately 30% of total protein mass. It forms the structural scaffold of tendons, ligaments, cartilage, bones, and skin — essentially everything that gives the foot its mechanical integrity. Understanding how collagen functions and deteriorates explains many of the foot conditions that develop with age and how to support optimal collagen health.
Collagen in Foot Tendons and Ligaments
The Achilles tendon, plantar fascia, and foot and ankle ligaments are predominantly type I collagen fibers organized in parallel bundles for tensile strength. As we age — and in response to repetitive loading beyond the tissue’s adaptive capacity — collagen fibers disorganize, cross-links break down, and the characteristic high-strength, organized structure of healthy tendon is replaced by disorganized fibrocartilaginous tissue (tendinosis). This is the underlying pathology of chronic plantar fasciitis and Achilles tendinopathy — not simply inflammation but structural collagen degeneration.
Collagen Synthesis: What the Body Needs
Collagen synthesis requires specific nutritional cofactors. Vitamin C is essential — it is the cofactor for prolyl and lysyl hydroxylase enzymes that cross-link collagen molecules. Without adequate vitamin C, newly synthesized collagen cannot form stable cross-links. Glycine (an amino acid abundant in bone broth and gelatin) is the most common amino acid in collagen and must be available for synthesis. Zinc and copper are additional cofactors for collagen maturation enzymes.
Collagen Supplementation: What the Evidence Shows
Hydrolyzed collagen peptide supplementation (10-15g daily, taken with vitamin C 30-60 minutes before exercise) has been studied in randomized controlled trials for tendon and ligament health. A 2019 study in the British Journal of Sports Medicine showed that collagen supplementation with vitamin C increased collagen synthesis markers and improved collagen structure in the patellar tendon following a standardized exercise protocol. The evidence for benefit in tendinopathy is promising but evolving — collagen supplementation is a reasonable addition to conservative tendinopathy management, not a replacement for exercise rehabilitation.
Lifestyle Factors That Damage Collagen
Smoking dramatically impairs collagen synthesis and is one of the most significant modifiable risk factors for poor wound healing and tendon repair after surgery. Elevated blood sugar (in diabetes and prediabetes) causes glycation — sugar molecules binding non-enzymatically to collagen fibers, stiffening them and reducing their mechanical properties (this contributes to the limited joint mobility seen in long-standing diabetes). Chronic NSAID use may impair tendon collagen healing by blocking the prostaglandin pathways that stimulate collagen synthesis.
