Swelling and Edema
Physiological dependent edema affects up to 75% of pregnant women, typically worsening in the third trimester as the enlarging uterus compresses pelvic veins and reduces venous return. For most women, this is a normal finding — not pathological edema. Management includes frequent foot elevation (above heart level when supine), compression stockings (Class 1, 15–20 mmHg, applied before arising from bed when edema is least significant), limiting prolonged standing, and maintaining regular low-impact activity. Sudden onset of severe unilateral leg swelling with calf pain requires immediate evaluation for deep vein thrombosis, which has a 5-fold increased incidence during pregnancy.
