Common Causes of Foot Pain by Location
Heel pain is most commonly caused by plantar fasciitis (pain on the bottom of the heel, worst with first steps), heel spurs, Achilles tendinopathy (pain at the back of the heel), retrocalcaneal bursitis, stress fracture, or Baxter’s nerve entrapment.
Ball of foot pain (forefoot) is most often caused by Morton’s neuroma, metatarsalgia, sesamoiditis, plantar plate tear, stress fracture of a metatarsal, or intermetatarsal bursitis.
Arch pain may result from plantar fasciitis (at the medial heel), posterior tibial tendon dysfunction (flatfoot), accessory navicular, navicular stress fracture, or Lisfranc ligament injury.
Toe pain can be caused by hallux rigidus (stiff big toe), bunion, ingrown toenail, hammertoe, mallet toe, claw toe, sesamoiditis, gout (acute attack at the first MTP joint), or toe stress fracture.

Physical examination includes gait analysis, range of motion measurement, neurovascular assessment, and specific provocative tests. In-office diagnostic ultrasound provides real-time visualization of soft tissue structures including tendons, ligaments, fascia, and nerves. Weight-bearing X-rays are performed in office during the first visit. Advanced imaging (MRI, CT) is ordered when the diagnosis requires confirmation or surgical planning is needed.
