When Routine Foot Care Isn’t Enough

Not all foot care is routine. If you have diabetes, vascular disease, neuropathy, a history of foot wounds, or other systemic conditions, your feet require specialized “high-risk” foot care. Who Is a High-Risk Foot Patient?

You’re considered a high-risk foot patient if you have: diabetes with neuropathy (loss of sensation), peripheral arterial disease or other vascular conditions, a history of foot ulcers or lower extremity amputation, kidney disease or dialysis dependence, immunosuppression from medications or illness, cancer or chemotherapy affecting immunity, blood thinning medications (warfarin, Eliquis, Xarelto), significant obesity, or age over 70 with reduced circulation and sensation.

What High-Risk Foot Care Includes at Our Office

High-risk patients receive a higher level of care at every visit. Our comprehensive high-risk protocol includes: thorough vascular assessment (pulses, capillary refill, skin temperature), neurological testing for sensation loss, complete skin and nail inspection, professional nail trimming with sterile instruments, callus and corn debridement (which can become ulcers if improperly managed), identification of pre-ulcerative changes before they break down, pressure redistribution with custom 3D-printed orthotics, diabetic shoe fitting (Medicare-covered for qualifying patients), wound care if breakdown has already occurred, MLS laser therapy for healing and circulation, and coordination with your other healthcare providers.

Why High-Risk Patients Should Never Go to a Nail Salon

This is one of the most important messages we share with our patients. Nail salons are not equipped to handle high-risk feet. They cannot assess for circulation problems or sensation loss, may use improperly sterilized tools that introduce infection, cannot identify pre-ulcerative skin changes, may cut nails improperly and cause injury, and cannot provide the clinical follow-up needed when a problem is identified. A cut that would heal in days for a healthy patient can become a limb-threatening infection for a diabetic or vascular patient.

Insurance Coverage

Medicare covers regular podiatric nail care for patients with qualifying high-risk conditions including diabetes with neuropathy, peripheral arterial disease, and other documented systemic conditions. Most Michigan private insurers follow similar guidelines.
