The Most Common Cause: Benign Nocturnal Leg Cramps
True nocturnal leg cramps are sudden, involuntary contractions of the calf or foot muscles that occur during sleep or rest. They’re extremely common — affecting up to 60% of adults at some point. Common triggers include dehydration (especially in warm weather or after exercise), electrolyte imbalances (low magnesium, potassium, or calcium), prolonged sitting or poor posture during the day, certain medications (diuretics, statins, beta-blockers, and others), and overexertion or unaccustomed exercise.
When Nighttime Leg Pain Signals Vascular Disease
The key distinction is between true cramps (sudden muscle spasm) and vascular rest pain. Peripheral arterial disease in its more advanced stage causes ischemic rest pain — a burning or aching in the feet and lower legs that occurs at night when circulation to the extremities decreases. Unlike muscle cramps, rest pain: is constant rather than spasmodic, is relieved by hanging the leg off the bed or standing (gravity assists blood flow), occurs in the feet and forefoot rather than just the calf, and is associated with other PAD signs (cold feet, color changes, slow healing).
Foot Cramps Specifically
Foot cramps — painful contractions of the arch or toe muscles — are often caused by overpronation (flat arches that strain the plantar muscles), ill-fitting footwear, neuropathy (nerve dysfunction causes abnormal muscle firing), and electrolyte deficiency.
