Foot and Ankle Problems Caused by Leg Length Discrepancy

When prescribing heel lifts, we generally correct to 50–75% of the measured discrepancy rather than full correction, especially for discrepancies over 10 mm. Overcorrection introduces new compensatory patterns—full correction of a long-standing 20 mm LLD may cause lateral hip pain on the now-elevated short side, knee pain from the altered tibial alignment, or Achilles tightness from the sudden reduction in plantarflexion compensation. Gradual titration—adding 3–5 mm at a time over several weeks—allows the soft tissues to adapt.
