Quick Answer

Most foot and ankle problems respond to conservative care — proper footwear, supportive inserts, activity modification, and targeted stretching — within 4-8 weeks. Persistent pain beyond that window, or any symptom that prevents walking, warrants a podiatric evaluation to rule out fracture, tendon tear, or systemic cause.

Regional anesthesia — specifically nerve blocks targeted to the foot and ankle — has become standard practice for foot and ankle surgery, providing superior post-operative analgesia compared to general anesthesia alone, reducing opioid requirements, and enabling enhanced recovery pathways that allow earlier ambulation and discharge. Understanding the different block options helps patients know what to expect and helps surgeons optimize their analgesic plans.

Digital Nerve Blocks

Digital nerve blocks anesthetize individual toes by blocking the common and proper digital nerves at the base of the toe. A bifurcation technique deposits local anesthetic (1% lidocaine without epinephrine, or 0.5% bupivacaine for longer duration) at the medial and lateral aspects of the metatarsal head — avoiding intraosseous injection. Epinephrine-containing solutions are contraindicated for digital blocks due to the risk of digital artery vasospasm and ischemia, though this traditional concern has been challenged by evidence that dilute epinephrine concentrations are likely safe in healthy digits. Digital blocks provide excellent anesthesia for ingrown toenail procedures, distal phalanx surgery, and minor forefoot procedures with 2–6 hour duration depending on agent selection.

Ankle Block

The ankle block provides complete foot anesthesia by blocking five nerves at the ankle level: the posterior tibial nerve (medial retromalleolar), sural nerve (lateral retromalleolar), superficial peroneal nerve (anterior ankle medial), deep peroneal nerve (anterior ankle lateral between EHL and EDL tendons), and saphenous nerve (anteromedial ankle). The ankle block is well-suited for forefoot and midfoot procedures — it has limited utility for hindfoot and ankle procedures because the surgical field is within or proximal to the block level. Ultrasound guidance significantly improves accuracy and reduces total anesthetic volume, decreasing the risk of local anesthetic systemic toxicity.

Popliteal Sciatic Nerve Block

The popliteal sciatic block — performed at the popliteal fossa where the sciatic nerve divides into the tibial and common peroneal nerves — provides anesthesia for the entire foot and ankle, making it ideal for hindfoot, rearfoot, and ankle procedures. Combined with a saphenous nerve block (for medial ankle coverage), the popliteal sciatic block provides complete below-knee anesthesia. Ultrasound-guided popliteal sciatic blocks using long-acting agents (0.5% bupivacaine or ropivacaine with or without dexamethasone adjuvant for extended duration) can provide 18–24 hours of post-operative analgesia — dramatically reducing immediate post-operative opioid requirements. Dr. Biernacki at Balance Foot & Ankle uses regional anesthesia protocols for appropriate surgical procedures to optimize patient comfort and post-operative recovery.
