The One Product That Helps Almost Every Foot Condition

If you could only add one product to address foot pain — and it had to work for the widest possible range of conditions — what would it be? Our podiatrists at Balance Foot & Ankle agree: it’s a quality arch support orthotic. Here’s why, and what makes it so universally effective.

The Case for Orthotics as the Universal Foot Product

The human foot has 26 bones, 33 joints, and over 100 tendons, muscles, and ligaments — all working together in a highly interconnected system. When the foundation (the arch) is misaligned, the mechanical stress doesn’t stay in the arch. It travels up through the ankle, knee, hip, and spine, causing pain at multiple points that might seem unrelated.

An arch support orthotic corrects the foundational alignment. When the foundation is right, the entire kinetic chain works more efficiently — and a surprising number of pain conditions improve or resolve.

Conditions That Respond to Orthotics

Plantar Fasciitis

The most well-documented application. Orthotics reduce tension on the plantar fascia by supporting the medial arch, reducing the stretch that causes heel pain. Multiple clinical studies confirm orthotics significantly reduce plantar fasciitis pain. This is the condition orthotics were originally designed for.

Ball of Foot Pain (Metatarsalgia)

Orthotics with metatarsal pads redistribute weight away from painful metatarsal heads. This is the second most common application our patients use orthotics for — often after years of undiagnosed “burning” or “walking on rocks” sensations in the ball of the foot.

Shin Splints

Overpronation (flat feet) creates excessive tibial rotation that stresses the periosteum of the tibia — causing shin splints. Correcting pronation with orthotics eliminates this mechanical stress. Many runners see immediate improvement.

Knee Pain (Runner’s Knee / Patellofemoral Pain)

Flat feet cause internal tibial rotation, which in turn causes the patella to track improperly. Correcting arch mechanics with orthotics can significantly reduce anterior knee pain in pronators — even when the knee itself has no structural problem.

Bunion Pain (Without Reversing the Bunion)

Orthotics reduce the pressure and friction forces on the first metatarsophalangeal joint that cause bunion pain and accelerate bunion progression. They won’t correct a bunion, but they slow its progression and often significantly reduce daily pain.

Achilles Tendinopathy

A heel lift component in an orthotic reduces the load on an inflamed Achilles tendon during walking. Combined with a slight arch correction, orthotics are often recommended as first-line conservative treatment for Achilles tendinopathy.

Flat Feet (Adult Acquired Flatfoot)

When the arch progressively collapses due to posterior tibial tendon dysfunction, orthotics support the collapsing medial column and significantly reduce pain and fatigue. Early orthotic use can slow progression of adult-acquired flatfoot.

Morton’s Neuroma

A metatarsal dome (a small pad placed under the metatarsal heads) built into an orthotic spreads the metatarsals apart, reducing compression of the nerve that causes Morton’s neuroma pain. Many patients achieve 50–70% pain relief from orthotics alone.

Why Orthotics Work So Broadly

Most foot problems share a common thread: abnormal mechanical stress. Whether it’s too much pressure in one area, excessive pronation creating rotational forces, or flat feet causing tendon overload, abnormal mechanics are the root cause. Orthotics address the root cause — not just the symptoms. That’s why they’re effective across such a many diagnoses.

OTC vs. Custom — Which Do You Need?

For most people with mild to moderate arch problems, a quality OTC orthotic ($30–80) is an excellent starting point. Our clinical experience: approximately 60–70% of patients who need orthotics see sufficient improvement from high-quality OTC options. The remaining 30–40% have structural needs that require custom-fabricated devices. Here’s how to know which category you fall into:

Start with OTC if: You have mild to moderate arch pain, plantar fasciitis under 6 months, or flat feet with no significant deformity. Most people respond well to quality OTC insoles.

Go custom if: You’ve tried 2–3 quality OTC insoles for 8+ weeks without improvement, you have a significant structural deformity (severe flat feet, severe high arches), you have diabetes or neuropathy, or your pain affects a body part up the chain (knee, hip, back) significantly.

Our Top OTC Orthotic Recommendations (2026)

Not all OTC insoles are created equal. These are the brands we actually recommend to patients:

🥇 #1 Best Overall: PowerStep Pinnacle
The most consistently effective OTC orthotic we’ve used across patient populations. The semi-rigid polypropylene arch shell provides genuine structural support (not just cushioning), the dual-layer foam maintains firmness over months of daily use, and it fits the widest range of shoe types. The Pinnacle is our first recommendation for plantar fasciitis, flat feet, and general arch support. Available at major retailers. [AFFILIATE LINK — PowerStep Pinnacle]

🥈 #2 Best for Active/Running Use: Curex RunPro
Curex takes a different approach: adaptive arch technology designed to respond to your movement rather than providing a fixed rigid profile. For patients who are runners, walkers, or need support during dynamic activity, the Curex RunPro provides support that feels more natural under load. Available in three arch heights (low, medium, high) to match your specific anatomy. [AFFILIATE LINK — Curex RunPro]

🥉 #3 For High Arches: PowerStep Pinnacle
If you have a notably high, rigid arch and standard insoles leave a gap at your arch, PowerStep Pinnacle’s aggressive arch profile fills that space. Works best for high-arch patients specifically. [AFFILIATE LINK — PowerStep Pinnacle]

When to See a Podiatrist Regardless of Products

Orthotics — even the best custom devices — are tools, not cures. If you have a condition requiring diagnosis, clinical intervention, or surgical evaluation, products are not a substitute. See a podiatrist if you have: pain that has lasted more than 8–12 weeks, open wounds or ulcers (especially with diabetes), significant deformity, or pain that is getting progressively worse despite using quality support products.
