Peripheral artery disease (PAD) — narrowing of the arteries to the legs and feet from atherosclerosis — affects approximately 8–12 million Americans, yet remains significantly underdiagnosed. The feet are often the first place PAD manifests with visible and palpable changes, making regular podiatric evaluation an important component of PAD management.
How PAD Affects the Feet
PAD reduces blood flow to the lower extremities, impairing the delivery of oxygen and nutrients to foot tissues. The clinical consequences range from mild exercise-related symptoms to life-threatening limb ischemia:
- Intermittent claudication — cramping, aching, or fatigue in the calf, thigh, or buttock that occurs predictably with walking a certain distance and resolves with rest; the “walking distance” decreases as disease progresses
- Rest pain — severe, burning pain in the foot and toes at rest (particularly at night or with leg elevation) indicates critical limb ischemia and requires urgent vascular evaluation
