Peripheral artery disease (PAD) — narrowing of the arteries to the legs and feet from atherosclerosis — affects approximately 8–12 million Americans, yet remains significantly underdiagnosed. The feet are often the first place PAD manifests with visible and palpable changes, making regular podiatric evaluation an important component of PAD management.

How PAD Affects the Feet

PAD reduces blood flow to the lower extremities, impairing the delivery of oxygen and nutrients to foot tissues. The clinical consequences range from mild exercise-related symptoms to life-threatening limb ischemia:

  • Intermittent claudication — cramping, aching, or fatigue in the calf, thigh, or buttock that occurs predictably with walking a certain distance and resolves with rest; the “walking distance” decreases as disease progresses
  • Rest pain — severe, burning pain in the foot and toes at rest (particularly at night or with leg elevation) indicates critical limb ischemia and requires urgent vascular evaluation
  • Skin changes — loss of hair on the foot and lower leg, shiny taut skin, muscle atrophy, and dependent rubor (redness when the foot is lowered, pallor when elevated) are hallmarks of chronic ischemia
  • Cool foot temperature — palpably cooler foot compared to the proximal leg or the contralateral limb
  • Absent or diminished pedal pulses — the dorsalis pedis and posterior tibial pulses are the primary clinical examination findings
  • Slow-healing wounds and ulcers — any wound on an ischemic foot heals poorly or not at all; even minor trauma can produce a non-healing ulcer
  • Gangrene — in end-stage critical limb ischemia, tissue death begins in the toes; wet gangrene with infection requires emergency amputation

Risk Factors for PAD

The strongest risk factors are: smoking (the single most modifiable risk factor), diabetes, hypertension, hyperlipidemia, older age, and a personal or family history of cardiovascular disease. Patients with diabetes and PAD together are at exceptionally high risk for limb loss.

Podiatric Role in PAD Management

Podiatrists serve a critical role in PAD management: performing vascular screening (ABI — ankle-brachial index — measurement), identifying at-risk feet before wounds develop, providing nail and callus care safely in patients where improper self-care carries high wound risk, prescribing protective therapeutic footwear, and coordinating with vascular surgery for patients with critical limb ischemia. Regular podiatric visits are recommended for all patients with known PAD.
