What Is the Plantar Plate?
The plantar plate is a thick fibrocartilaginous structure on the underside of each MTP joint (the knuckles at the ball of the foot). It acts as the primary stabilizer preventing the toes from dorsiflexing excessively and maintaining them in a plantigrade position. The second MTP joint plantar plate is the most commonly injured, likely because of its anatomically vulnerable position — the second metatarsal is often the longest and bears more load during push-off.

Symptoms of a Plantar Plate Tear
The distinguishing feature of plantar plate injury is a combination of pain AND instability. Specifically: pain under the second or third MTP joint (not between the toes like a neuroma), a “V sign” — widening gap between the second and third toes as the second drifts medially, toe that floats upward or crosses over the big toe in advanced cases, pain with the “drawer test” (pushing the toe upward reproduces plantar symptoms), swelling at the ball of the foot, and pain that worsens with barefoot walking, high heels, or any activity requiring toe push-off.
What Causes Plantar Plate Tears?
Acute tears occur from sudden hyperextension of the toe — stepping off a curb wrong, or catching a toe during sport. Chronic tears develop gradually from repetitive overloading: high-heeled shoes (push the MTP joints into constant hyperextension), a long second metatarsal creating excess forefoot loading, hallux valgus (bunion) causing the big toe to crowd the second and transfer load, and inflammatory arthritis weakening the ligament over time.

Plantar Plate Tear or Morton’s Neuroma? This Is the Distinction That Gets Missed
More people arrive here having already been treated for a neuroma than for anything else. The two conditions produce forefoot pain in overlapping territory, and they are managed in almost opposite ways — so getting the label right is not academic.
The most reliable separator is where the tenderness actually is. Press between the metatarsal heads, in the soft web space. Then press directly underneath the joint at the base of the toe, on the bone. Neuroma pain lives in the web space and typically radiates outward into the two adjacent toes, often with numbness, tingling or the sensation of a bunched-up sock. Plantar plate pain sits under the joint itself, is usually accompanied by swelling on the top of that joint, and is far more often described as a deep ache or a feeling that the toe is loose than as an electric or burning pain.
The second separator is what the toe is doing over time. A neuroma does not move your toe. A failing plantar plate does — the toe begins to sit slightly higher, then drifts toward its neighbour, and eventually crosses over it. That drift is the plantar plate telling you it is no longer holding the joint down.
The drawer test, which settles it in about ten seconds
This is the single most useful thing done in the examination room for this diagnosis. The metatarsal is held steady with one hand while the base of the toe is pushed upward with the other. On a healthy joint there is essentially no give. Where the plantar plate has torn, the toe translates upward out of the joint — and how far it goes is what grades the injury, from mild laxity through partial subluxation to a joint that can be fully dislocated by hand.
It matters because the grade drives the plan. Early laxity responds well to taping and offloading. A joint that subluxes more than halfway, or a toe that has already crossed over, is far less likely to be talked back into place without surgical repair — and knowing that at the first visit prevents a year spent on treatment that was never going to work.
The paper pull-out test you can do at home
Slide a strip of paper under the affected toe while standing, press the toe down onto it as hard as you can, and have someone pull the paper out. Repeat with the same toe on the other foot. A healthy toe grips hard enough to tear the paper or hold it firmly. If the affected side lets go with noticeably less resistance, that toe is not able to press down properly — which is exactly what a plantar plate does for a living. It is not a diagnosis, but it is a good reason to book the examination.
Why a Cortisone Shot Can Make a Plantar Plate Tear Worse
This deserves its own section, because it is the most consequential thing on this page.
When forefoot pain is assumed to be a neuroma, a steroid injection is a common and often reasonable next step. When the actual problem is a plantar plate tear, that same injection is working against you. Corticosteroid weakens collagenous tissue, and the plantar plate is a collagenous ligament already under strain and already partly torn. Injecting steroid in and around it can accelerate the failure — and the classic sequence patients describe is a few good weeks followed by a toe that starts visibly drifting or lifting, which is a deformity that does not reverse on its own.
None of this makes injections bad medicine. It makes the diagnosis worth ten seconds of a drawer test beforehand. If you have been offered an injection for forefoot pain and the toe has been changing position, that is a reasonable moment to ask directly whether the plantar plate has been assessed.
Imaging: what actually shows a tear
Plain X-rays do not show the plantar plate, because it is soft tissue. They are still worth taking — they show the toe’s alignment, rule out a stress fracture presenting as the same forefoot pain, and reveal the long second metatarsal that predisposed the joint in the first place. But a normal X-ray does not mean nothing is torn, and being told “the X-ray was fine” is another common route to a missed diagnosis.
Ultrasound is inexpensive, immediate, and has the distinct advantage of being dynamic — the joint can be stressed while it is being watched. MRI gives the most detailed picture of the plate itself and of the surrounding structures, and is what most surgeons want before planning a repair.
Getting the Conservative Treatment Right, Because Most People Do Not
Non-surgical management of a low-grade tear works genuinely well, and it fails far more often from execution than from the plan being wrong. Three details account for most of that.
The metatarsal pad goes behind the metatarsal head, not under it. This is the single most common mistake, and it converts a helpful pad into a painful one. The purpose of the pad is to lift and spread the metatarsals just before the sore joint so that pressure is taken off it. Placed directly beneath the tender spot, it simply presses harder on the thing that hurts. If a pad made your forefoot pain worse, it was almost certainly too far forward rather than the wrong idea.
The taping has to hold the toe pointing slightly downward, and it has to stay on. The tape is doing the plantar plate’s job while the tissue tries to scar down, so the toe is strapped in a mildly plantarflexed position — pulled gently down toward the floor, not pulled straight or up. Taping a toe in a neutral position achieves close to nothing.
It takes far longer than people expect. Ligament heals slowly and the forefoot is loaded with every single step, so meaningful recovery is measured in months — commonly eight to twelve weeks of consistent offloading before the joint is reliably comfortable, and often longer before high-impact activity returns. The most common reason a plantar plate protocol “did not work” is that it was abandoned at three weeks. A stiff-soled shoe or a carbon plate over that period does more good than any single intervention, because it stops the toe joint from bending at push-off at all.
Barefoot walking on hard floors, flexible shoes that fold in half, and going back to running early are the three things that most reliably undo the progress.
