What Is Soft Tissue Sarcoma?
Soft tissue sarcomas are malignant tumors arising from connective tissue — muscle, fat, fibrous tissue, blood vessels, or nerves. They account for about 1% of adult cancers and can occur anywhere in the body, including the foot and ankle. The foot is an uncommon but well-documented location.
Common Types in the Foot
Types found in the foot include synovial sarcoma (most common in the foot/ankle region), epithelioid sarcoma, clear cell sarcoma (“melanoma of soft parts”), fibrosarcoma, and undifferentiated pleomorphic sarcoma. Synovial sarcoma, despite its name, does not arise from synovium — it can occur anywhere and is the most common sarcoma in the distal extremities.
Warning Signs: “Rule of 3” for Suspicious Masses
Any foot mass should be evaluated urgently if it: (1) is larger than 5 cm (golf ball size), (2) is deep to the fascia (not freely mobile in the superficial skin layer), or (3) is growing. A mass that is painful can be benign or malignant — the absence of pain does not exclude sarcoma.
Distinguishing From Benign Tumors
The foot commonly hosts benign masses including lipomas, ganglion cysts, plantar fibromas, and inclusion cysts. These are usually soft, superficial, or have characteristic features on ultrasound. Any atypical features warrant MRI and specialist referral rather than reassurance.
Diagnosis
MRI with and without contrast is the primary imaging modality for soft tissue masses — it characterizes size, depth, and proximity to neurovascular structures. CT may show calcifications (30% of synovial sarcomas). Biopsy is mandatory — core needle biopsy by an orthopedic oncologist is the standard approach. Never excise an undiagnosed mass without imaging; “shell-out” excision of a sarcoma seeds the wound and dramatically worsens prognosis.
Treatment
Limb-sparing surgery with wide surgical margins is the primary treatment. Radiation therapy (pre- or post-operative) reduces local recurrence for high-grade or close-margin cases. Chemotherapy is used for high-grade, large, or metastatic sarcomas. Foot sarcomas present unique challenges because of limited soft tissue coverage and the desire to preserve function.
Prognosis
Five-year survival rates vary widely by grade (low-grade: 90%+; high-grade: 50–60%) and stage. Synovial sarcoma of the distal extremity has relatively favorable prognosis compared to truncal sarcomas. Local recurrence after inadequate excision is a major concern.
