These three deformities look similar but involve different joints. Hammertoe is contracture of the proximal interphalangeal (PIP) joint — the middle toe joint flexes downward while the MTP joint (toe-foot connection) often extends upward. The dorsal (top) surface of the PIP joint and the tip of the toe bear friction against shoes. Mallet toe is isolated contracture of the distal interphalangeal (DIP) joint — the nail tip points downward, creating a mallet head appearance. Claw toe involves both PIP and DIP joint flexion with MTP joint extension — the most severe deformity, often associated with neurological conditions or severe flat foot.

Wait until it is rigid, and you need bone resection with a longer recovery. The other thing patients don’t realize: the surgery is far less dramatic than they imagine.
