This “idiopathic toe walking” is often habitual and may have a familial component. However, toe walking can also be a sign of: tight Achilles tendons or calf muscles (equinus contracture), which may be structural or the result of habitual toe walking itself; cerebral palsy or other upper motor neuron conditions causing spasticity; autism spectrum disorder, where sensory sensitivities may contribute to gait pattern; leg length discrepancy; or, rarely, muscular dystrophy. The distinction between idiopathic toe walking and neurologically driven toe walking is important for choosing the right treatment approach.
Treatment Options
For idiopathic toe walking in young children, watchful waiting is appropriate if the child can place their heels on the ground when asked and there is no significant Achilles tightness. If Achilles tightness is present, a stretching program is first-line treatment. For persistent toe walking with documented equinus contracture, serial casting (progressive plaster casts applied every few weeks to gradually lengthen the calf) is an effective non-surgical option. Ankle-foot orthoses (AFOs) may be used to maintain range of motion after serial casting or to support proper gait mechanics. In cases where conservative measures have been exhausted and significant equinus contracture persists, surgical lengthening of the Achilles tendon or gastrocnemius-soleus complex may be considered.
