Track and Field Foot Injuries: Sprinters Distance Runners and Jumpers Common Problems

Event-Specific Foot Demands in Track and Field

Track and field encompasses a remarkably diverse range of athletic disciplines — from the explosive 100-meter sprint to the grueling 10,000-meter run, from the precision of triple jump to the power of shot put. Each event creates a distinct mechanical load pattern on the foot and ankle, leading to injury profiles that differ substantially across disciplines. Sprint Events: 100m, 200m, and 400m

Sprinting demands the highest instantaneous force production of any running discipline. Elite sprinters generate ground reaction forces exceeding three times body weight with each stride, transmitted primarily through the forefoot and metatarsal heads in the spike contact pattern. This extreme forefoot loading drives two dominant injury patterns in sprinters.

Plantar plate tears and MTP joint injuries occur from the hyperextension loading of the big and lesser toe joints during push-off. Sprinters develop plantar plate stress and tears — chronic forefoot pain that worsens with push-off activities and resolves partially with rest, only to recur when sprint training resumes. Sesamoiditis and sesamoid stress fractures are also common from the intense sesamoid loading during the push-off phase of sprinting.

Hamstring and proximal hamstring insertional pain at the sitting bone is common in sprinters and typically involves the proximal hamstring attachment at the ischial tuberosity, but the Achilles tendon and plantar fascia absorb significant force during the acceleration phase when the ankle is in extreme dorsiflexion. Any forefoot pain in a sprinter that does not resolve with relative rest warrants imaging to exclude stress fracture.

Spike shoes for sprinting have rigid carbon fiber plates and minimal cushioning — designed for stiffness and energy return at the cost of cushioning. The extreme stiffness is appropriate for competition but makes prolonged spike training without alternating with training flats a significant overuse injury risk factor.

Distance Running Events: 1500m Through 10,000m

Distance track runners share most injury patterns with road runners but face the added demands of banked track surfaces, tight turns (particularly in indoor track events on 200m oval tracks), and the repetitive loop pattern that concentrates lateral forces on the outer lane foot. The banked indoor track creates a leg length discrepancy simulation — the inner leg is effectively shorter and the outer leg longer with each lap, driving asymmetric injury patterns when athletes train exclusively on indoor tracks.

Tibial and metatarsal stress fractures, plantar fasciitis, Achilles tendinopathy, and peroneal tendinopathy from the repetitive lateral loading on banked turns are the most common conditions in distance track specialists. Navicular stress fractures deserve special mention as a condition particularly affecting distance track athletes — the navicular is at risk from the repetitive loading of track turns combined with the higher training volume of distance events.

Jumping Events: High Jump, Long Jump, Triple Jump, Pole Vault

Jumping athletes experience some of the highest instantaneous joint loads in all of athletics. Triple jumpers are particularly vulnerable — the three sequential impacts of hop, step, and jump create cumulative loading on the heel, midfoot, and forefoot in rapid succession. Calcaneal stress fractures from the heel-dominant hop phase, plantar fascia tears from the explosive take-off, and ankle impingement from the landing phase are all characteristic of triple jump.

Pole vaulters develop unique foot problems from the high dorsiflexion demands of the plant phase and the landing impact from up to six meters of height. High jumpers developing Fosbury Flop mechanics sometimes develop medial ankle pain from the extreme supination and push-off forces of the one-footed curved approach.

All jumping athletes are at elevated stress fracture risk given the cyclic impact loading. Any localized bone pain in a jumping athlete requires imaging before return to competition, as a complete fracture during the explosive loading of jumping competition can be catastrophic.

Throwing Events: Shot Put, Discus, Hammer Throw, Javelin

Throwers may seem unlikely foot injury patients, but the rotational mechanics of discus and hammer throw place significant torsional forces on the foot and ankle. The spinning technique requires rapid weight transfers and extreme rotational loads through the planted foot, creating risk for ankle sprain, peroneal tendon injury, and turf toe-like loading of the push-off foot. Javelin throwers stress the cross-step plant foot during the approach, and the braking forces of the approach create significant heel and Achilles loading.

Prevention and Management

Event-specific footwear selection is critical. Spike shoes should match the event demands — distance spikes have more cushioning and flexibility than sprint spikes. Alternate spike training with training flat sessions to reduce the cumulative forefoot loading that drives stress injuries. Cross-train with low-impact activities during periods of high training volume to reduce bone stress while maintaining aerobic and muscular fitness.

Any localized bone pain that worsens with training and is consistent across multiple sessions requires imaging before it progresses to complete fracture. Early stress reactions heal within 4 to 6 weeks of relative rest; complete fractures may require 8 to 16 weeks and in some locations (navicular, fifth metatarsal) benefit from surgical fixation for faster return to competition.
