What Is Anterior Ankle Impingement?

Anterior ankle impingement — also called “footballer’s ankle” — occurs when bone spurs (osteophytes) or soft tissue proliferations on the anterior (front) aspect of the distal tibia and neck of the talus collide during ankle dorsiflexion, causing pain in the front of the ankle. Every time the foot flexes upward — stair climbing, squatting, deep ankle dorsiflexion in sports — the anterior osteophytes meet and pinch soft tissue between them. The result is deep, aching anterior ankle pain during these activities, often with a sensation of the ankle “catching” or limited dorsiflexion range. Causes — Athletic Overuse and Previous Injury

Anterior ankle impingement develops from repetitive microtrauma to the anterior ankle: soccer players developing tibiotalar osteophytes from repeated ball striking with maximal plantarflexion (the original “footballer’s ankle” population); gymnasts and ballet dancers who repeatedly dorsiflexed maximally at the ankle joint; basketball and football players with multiple anterior ankle sprains over time; and distance runners with high training volume. Each episode of microtrauma stimulates periosteal bone formation at the anterior tibia and dorsal talus — the body’s adaptive response to repetitive stress. Over years, these osteophytes grow large enough to create mechanical impingement during normal dorsiflexion range.

Diagnosis

Anterior impingement produces pain specifically with forced dorsiflexion (not with plantarflexion) — the forced dorsiflexion impingement test reproduces pain in the anterior ankle. Tenderness is located at the anterior joint line, not over the ligaments. Weight-bearing lateral X-ray clearly shows anterior tibial and dorsal talar osteophytes — their size and proximity on lateral X-ray predicts the degree of impingement. CT scan quantifies osteophyte dimensions and their relationship to the joint. MRI identifies associated soft tissue impingement (synovial thickening, scar tissue in the anterior gutter) and any osteochondral lesion of the talar dome that may be contributing to pain.

Conservative Management

Conservative management for anterior ankle impingement aims to reduce activity-related dorsiflexion demand: heel lift inserts (10–12mm) that reduce the ankle dorsiflexion angle required for a given activity; activity modification to avoid deep squats, uphill running, and stairs as tolerated; and physical therapy to maximize available soft tissue mobility around the osteophytes. Corticosteroid injection into the anterior ankle gutter reduces synovial thickening and soft tissue impingement component — provides temporary relief but does not address the mechanical bone impingement. Conservative management provides incomplete relief in most cases with established osteophytes — the mechanical impingement does not resolve without removing the osteophytes.

Arthroscopic Cheilectomy — Surgical Treatment

Arthroscopic anterior ankle cheilectomy — removal of the anterior tibial and dorsal talar osteophytes through small arthroscopic portals — is the definitive treatment for anterior ankle impingement. The procedure is performed as an outpatient under regional or general anesthesia; the osteophytes are resected with a shaver or burr under direct arthroscopic visualization. Outcomes: 80–90% patient satisfaction; restoration of full dorsiflexion; return to full athletic activity at 6–10 weeks. The recurrence rate depends on continued participation in the same high-dorsiflexion activities — athletes who continue soccer or gymnastics have higher osteophyte recurrence rates over 5–10 years compared to recreational athletes.
