What Causes Foot Blisters?
Blisters form when repetitive friction causes the outer layers of skin (epidermis) to separate, creating a fluid-filled pocket. The fluid (serum) cushions the underlying tissue. Three factors must be present: friction, moisture, and time. Remove any one of these, and blisters don’t form.
Common specific causes:
- New shoes without proper break-in
- Shoes that are too large (causing slippage) or too small (causing pressure)
- Wet feet — moisture dramatically increases skin friction coefficient
- Cotton socks — cotton absorbs moisture and holds it against the skin
- High-friction areas: heel counter, toe box seams, ball of foot
- Sudden increase in mileage or activity without gradual adaptation
Blister Prevention: What Actually Works
1. Moisture-Wicking Socks (NOT Cotton)
Merino wool or synthetic performance socks (Darn Tough, Balega, Drymax) are dramatically superior to cotton for blister prevention. They wick moisture away from the skin surface, reducing the friction coefficient. 2. Proper Shoe Fit
Shoes should have a thumb-width of space at the toe and zero heel slippage. Buy shoes in the afternoon (feet swell throughout the day). Break in new shoes gradually — never wear new footwear for a major event without prior use.
3. Lubricants and Anti-Friction Products

How to Treat a Blister Correctly
Small Blisters (Under 5mm) — Leave Intact
The blister roof is a sterile, protective covering over the damaged tissue. Do not pop small blisters. Cover with a blister bandage and reduce friction at the site.
Large, Painful Blisters — Drain, Don’t Remove the Roof
- Clean the skin with alcohol or betadine
- Use a sterile needle (alcohol-flamed or from a sterile pack) to puncture the blister at its edge
- Gently express the fluid by pressing — do not tear the roof
- Apply antibiotic ointment (bacitracin)
- Cover with a non-stick blister bandage or Tegaderm
Never remove the blister roof — it provides vital protection against infection.
When to See a Podiatrist
