Why Pregnancy Changes Your Feet
Medically Reviewed by Dr. Tom Biernacki, DPM — Board-Certified Podiatrist, Balance Foot & Ankle Specialists, Michigan. Last updated April 2026.
Pregnancy produces profound physiologic changes that affect every system in the body, and the feet are no exception. Hormonal shifts, progressive weight gain, altered center of gravity, and changes in circulation and connective tissue all impact foot structure and function during the 40 weeks of gestation and beyond. Understanding these changes helps expectant mothers protect their foot health and seek appropriate care when needed.
Foot Swelling (Edema) in Pregnancy
Physiologic edema—generalized swelling from increased blood volume and venous pressure in the legs—affects the majority of pregnant women, particularly in the third trimester. Feet and ankles are most visibly affected. The swelling typically worsens after prolonged standing or in warm weather and improves overnight with elevation. Management includes elevating the legs above heart level for 20–30 minutes several times daily, wearing graduated compression stockings (15–20 mmHg for pregnancy), reducing sodium intake, and maintaining gentle exercise such as walking and swimming to promote venous return. Sudden, severe, or one-sided swelling warrants immediate medical evaluation to exclude deep vein thrombosis or preeclampsia.
Arch Changes and Plantar Fasciitis
Relaxin—a hormone produced in increasing amounts during pregnancy—loosens the ligaments throughout the body to prepare the pelvis for childbirth. This ligamentous laxity also affects the foot, allowing the arch to flatten more than usual under increased body weight. The result is increased plantar fascia tension and, in many women, the development of plantar fasciitis—sharp heel pain that is worst with the first steps in the morning or after rest periods. Supportive footwear with substantial arch support, off-the-shelf or custom orthotics, and regular calf and plantar fascia stretching are safe and effective during pregnancy.
Permanent Shoe Size Changes
Research demonstrates that pregnancy causes measurable, permanent changes in foot length and arch height in a significant percentage of women. A study published in the American Journal of Physical Medicine and Rehabilitation found that the arch height of first-time mothers decreased and foot length increased during pregnancy and remained changed postpartum. Many women notice they can no longer comfortably wear their pre-pregnancy shoe size after delivery. This is a structural rather than a fluid-related change and persists indefinitely.
Ingrown Toenails During Pregnancy
Pregnancy-related foot swelling and pressure changes in footwear can cause or exacerbate ingrown toenails. The great toenail is most commonly affected, producing pain and potential infection if the nail border penetrates the surrounding skin. Podiatric treatment of ingrown toenails is safe during pregnancy. Nail edge removal under local anesthesia provides immediate relief; permanent nail ablation with phenol is avoided during the first trimester but is generally considered acceptable in the second and third trimesters with physician coordination.
What Footwear Is Best During Pregnancy?
Pregnancy footwear should prioritize comfort, stability, and arch support. Choose shoes with a wide toe box that accommodates swelling, substantial medial arch support (not flat sandals or flip-flops, which exacerbate arch strain), a low heel of no more than one inch, adjustable fastening (laces or Velcro) to accommodate daily swelling fluctuation, and non-slip soles for safety as center of gravity shifts. High heels and unsupportive flats are contraindicated during pregnancy due to their contribution to arch strain and fall risk.
Postpartum Foot Care
Many women find that foot pain persists or even worsens in the early postpartum period when the demands of newborn care create prolonged periods of standing and walking on a body whose ligaments are still relaxin-lax. Postpartum podiatric evaluation is appropriate for any woman with persistent heel pain, arch pain, or foot deformity following pregnancy. Custom orthotics fitted after birth accommodate the permanently changed foot structure and provide durable symptom relief.
