Osteomyelitis — bacterial infection of bone — in the diabetic foot is a major driver of lower extremity amputation and is present in up to 20% of diabetic foot ulcers at initial presentation. Its recognition transforms wound management from local care to systemic antibiotic therapy, often with surgical debridement or bone resection — a management escalation that requires accurate diagnosis before it is initiated. Overdiagnosis leads to unnecessary surgery and prolonged antibiotic courses; underdiagnosis leads to progressive bone destruction, treatment failure, and amputation.
Clinical Diagnosis: The Probe-to-Bone Test
The probe-to-bone (PTB) test is the most clinically useful bedside test for osteomyelitis: a sterile metal probe is passed through the ulcer to assess depth and whether bone is palpable at the wound base. A positive PTB test (metal probe contacts hard, gritty bone) has a positive predictive value for osteomyelitis of 89% in a high-prevalence diabetic foot ulcer population (Grayson et al., JAMA 1995). A negative PTB in a low-risk wound (small, superficial, non-infected) has a high negative predictive value. Other clinical markers: an ulcer area > 2cmΒ² (correlates with osteomyelitis risk), visible or exposed bone in the wound base (PPV 80%), erythrocyte sedimentation rate (ESR) > 70mm/hr, and C-reactive protein (CRP) elevation suggesting systemic inflammation.
Imaging: X-Ray, MRI, and Bone Biopsy
Plain radiograph is the initial imaging: cortical destruction and periosteal reaction indicate established osteomyelitis — but these changes lag 10–21 days behind histological infection, making plain film insensitive for early disease. MRI (STIR sequence) is the definitive non-invasive imaging: bone marrow edema (STIR signal increase) at the cortex adjacent to the ulcer is 90% sensitive and 82% specific for osteomyelitis. The gold standard for definitive diagnosis is bone biopsy for histology (necrotic bone, inflammatory infiltrate, bacterial colonies) and tissue culture to identify the specific organism and guide targeted antibiotic therapy. Culture of superficial wound swabs is misleading — surface swab cultures reflect colonization, not the infecting organism in deep bone. Percutaneous CT-guided bone biopsy or surgical specimen from debridement provides the definitive specimen. Treating osteomyelitis based on surface swab culture is a major management error that produces antibiotic treatment failure.
Surgical and Medical Management
Surgical debridement — removal of infected and necrotic bone back to viable bleeding cortex — combined with antibiotic therapy produces the highest cure rates. “Medical management” with antibiotics alone (without surgery) is appropriate for: osteomyelitis without bone necrosis, the early MRI-confirmed case, and when surgical risk is prohibitive. Duration of antibiotic therapy: 4–6 weeks IV or highly bioavailable oral antibiotics (e.g., rifampin-based combinations, fluoroquinolones) for post-surgical management; 6–12 weeks for non-surgical management. Hyperbaric oxygen therapy is an adjunct in select cases with demonstrated tissue hypoxia.
