Not All Heel Pain Is Plantar Fasciitis
Heel pain is one of the most common complaints in athletic populations, but “plantar fasciitis” is applied as the default diagnosis far too broadly. Multiple conditions cause heel pain in athletes, and they require distinctly different treatments. Misdiagnosis leads to prolonged, ineffective management. At Balance Foot & Ankle in Howell and Bloomfield Township, Michigan, we perform precise diagnostic evaluation to ensure athletes receive targeted treatment.
Plantar Fasciitis: The Classic Presentation
Plantar fasciitis causes pain at the medial (inner) heel, at the fascia’s calcaneal insertion. The pain is worst with the first steps of the morning or after prolonged rest, and typically eases somewhat after warming up — though it may return after extended activity. Tenderness is localized to a specific point at the inner heel, directly at the fascia’s insertion. X-rays may show a heel spur (present in 50% of plantar fasciitis cases and 25% of asymptomatic heels — the spur itself is not the pain source).
Calcaneal Stress Fracture
Calcaneal stress fractures are frequently misdiagnosed as plantar fasciitis. Key differentiating features: a “squeeze test” (compressing the heel from both sides simultaneously) reproduces sharp pain in stress fractures but not plantar fasciitis. The pain location is often more diffuse and central within the heel body rather than the focal medial insertion point of plantar fasciitis. Pain worsens progressively through activity and doesn’t follow the post-rest pattern of plantar fasciitis. MRI is needed when stress fracture is suspected, as X-rays may be negative in early stages.
Sever’s Disease (In Adolescents)
In children and adolescents between 8 and 14, plantar fasciitis is uncommon — heel pain is far more likely to represent Sever’s disease (calcaneal apophysitis). The pain location is at the back of the heel (the Achilles insertion on the growth plate) rather than the bottom, and the heel squeeze test is positive over the posterior heel. Distinguishing these conditions prevents inappropriate plantar fasciitis treatment in children.
Insertional Achilles Tendinopathy
Pain at the back of the heel from Achilles tendon insertion pathology is sometimes confused with plantar fasciitis. The location distinguishes them: insertional Achilles pain is posterior (back of heel), plantar fasciitis is inferior-medial (bottom-inner heel). X-rays may show calcification within the Achilles insertion in insertional tendinopathy. Treatment approaches differ significantly between the two conditions.
Heel Pad Syndrome
Heel pad syndrome is central heel pain from atrophy or inflammation of the plantar fat pad. Unlike plantar fasciitis, the tenderness is diffuse and centered directly under the heel rather than at the medial margin. It is more common in older athletes and those with very high activity levels. Treatment focuses on cushioning rather than arch support.
