Tendinosis differs from tendinitis (acute inflammation) and indicates a longer-standing condition. Common locations include the Achilles tendon, posterior tibial tendon, and peroneal tendons.
Partial tear means some but not all fibers of a tendon or ligament are disrupted. Partial tears range from low-grade (less than 25 percent of fibers) to high-grade (more than 75 percent) and vary widely in clinical significance. Not all partial tears require surgery — many heal with conservative treatment.
Effusion refers to fluid within a joint space, indicating inflammation or injury. Small effusions are extremely common on foot MRI and often represent normal physiological fluid rather than a pathological process. Large effusions or effusions accompanied by other findings may indicate significant joint pathology.
Degenerative changes and osteoarthritis describe age-related joint wear including cartilage thinning, bone spur formation, and joint space narrowing. These findings are present on most adult foot MRI exams and frequently do not correlate with symptoms — meaning the arthritis visible on MRI may not be causing your pain.
MRI Findings That Often Need Treatment
Complete tendon tears with retraction require surgical consideration because the severed ends cannot heal without reapproximation. Complete Achilles tendon ruptures, posterior tibial tendon tears, and peroneal tendon tears typically produce significant functional deficits that conservative treatment alone cannot fully address.
Stress fractures confirmed by bone marrow edema patterns on MRI require activity modification and protected weight-bearing to allow healing. Navicular and fifth metatarsal stress fractures are particularly concerning because these bones have tenuous blood supplies that increase nonunion risk without appropriate treatment.
Osteochondral lesions of the talus — areas of damaged cartilage and underlying bone in the ankle joint — may require surgical intervention when they cause persistent pain, mechanical symptoms, or are large enough to threaten joint integrity. MRI characterizes the size, location, and stability of these lesions to guide treatment decisions.
Infections (osteomyelitis, septic arthritis) appear as aggressive bone marrow edema patterns with surrounding soft tissue inflammation. These findings require urgent treatment with antibiotics and potentially surgical debridement to prevent progressive bone destruction.
MRI Findings That Often Do Not Need Treatment
Incidental findings are abnormalities discovered on MRI that are unrelated to your symptoms. Up to 40 percent of foot MRI examinations reveal findings that exist but cause no symptoms. These incidentalomas can create unnecessary anxiety if patients assume every abnormality requires intervention.
Small joint effusions, minor tendon signal changes, and age-appropriate degenerative findings are extremely common and typically represent normal aging rather than clinical problems. A 50-year-old foot has accumulated decades of normal wear that appears as signal changes on MRI without necessarily producing any symptoms.
Bone marrow variations including islands of fatty marrow, residual red marrow, and small cysts (geodes) are normal variants that can appear concerning on MRI but require no treatment. The radiologist typically identifies these as normal variants in the report.
Ganglion cysts are benign fluid-filled sacs that appear frequently on foot MRI. Most are asymptomatic and discovered incidentally. Only ganglion cysts that cause pain, nerve compression, or functional limitation require treatment — the majority can be safely monitored.
