Do I Need a Referral to See a Podiatrist? (Insurance Guide 2026)

Most insurance plans do NOT require a referral to see a podiatrist — podiatry is a primary specialty, meaning you can self-refer. Plans that typically don’t require referrals: Medicare (Original), most PPOs (Blue Cross PPO, Aetna PPO, Cigna PPO, UnitedHealthcare PPO). Plans that often DO require referrals: HMOs (BCBS HMO, HAP HMO, Priority Health HMO, Medicare Advantage HMO), some Medicaid managed-care plans. Always call your insurance to confirm.

In my Michigan podiatry clinic, ~90% of new patients walk in WITHOUT a referral and have full coverage. If your plan requires one: (1) call your primary care doctor’s office to request a podiatry referral, (2) your PCP can usually fax/electronically send it within 24-48 hours, (3) same-week appointments are still possible. “Do I need a referral to see a podiatrist?” — This is one of the most frequently asked questions from new patients, and the answer depends on your insurance plan type. Here is a clear breakdown so you can get the foot care you need without unnecessary bureaucratic delays.

The Short Answer

Most patients do NOT need a referral. The majority of commercial insurance plans, Medicare, and Medicaid allow direct access to podiatric physicians without requiring a referral from a primary care physician. Exceptions exist primarily in HMO (Health Maintenance Organization) plans.

By Insurance Type

PPO (Preferred Provider Organization) — No Referral Needed

PPO plans allow you to see any in-network specialist, including podiatrists, directly — without obtaining a primary care referral first. This is the most common commercial insurance structure for employed individuals in Michigan. Simply verify that your podiatrist is in-network with your PPO and call to schedule directly.

HMO (Health Maintenance Organization) — Referral Usually Required

HMO plans typically require that you receive a referral from your designated primary care physician (PCP) before seeing a specialist. Contact your PCP office, explain your foot concern, and request a specialist referral to podiatry. Most PCP offices can process referrals within 1–3 business days.

Medicare Part B — No Referral Needed

Medicare beneficiaries can see a participating podiatrist directly without a referral for covered services (medically necessary foot conditions, diabetic foot care, etc.). Medicare Part B covers podiatric services at 80% of the Medicare-approved amount after the deductible; supplemental insurance covers the remaining 20%.

Medicaid (Michigan) — Varies by Plan

Michigan Medicaid (Healthy Michigan Plan) and Medicaid managed care plans vary. Straight fee-for-service Medicaid typically allows direct podiatric access. Managed care Medicaid plans may require PCP referral. Contact your specific Medicaid plan’s member services to confirm.

Medicare Advantage (Part C) — Varies by Plan

Medicare Advantage plans administered by private insurers (Blue Cross, Priority Health, HAP, etc.) have their own referral policies. Some require PCP referral; others allow direct specialist access.
