Cracked heels — also called heel fissures — are a common but frequently undertreated foot problem. When the dry, thickened skin around the heel rim splits, the cracks (fissures) can be shallow and cosmetically bothersome, or deep and painful enough to bleed and become infected. At Balance Foot & Ankle, Dr. Tom Biernacki treats heel fissures safely and effectively, with special attention to high-risk patients with diabetes, peripheral vascular disease, or neuropathy.

Why Do Heels Crack?

The heel bears enormous repetitive pressure. Without adequate moisture, the thick callus skin around the heel rim loses elasticity and splits when mechanically stressed. Contributing factors include: dry climate or dry indoor air, going barefoot or wearing open-back shoes (sandals, flip-flops, backless mules) that allow heel spreading under load, prolonged standing, obesity, hypothyroidism (causes dry skin throughout the body), eczema or psoriasis, fungal infection (athlete’s foot drying the skin), and vitamin deficiencies. In diabetic patients, autonomic neuropathy impairs sweat production in the feet, making skin chronically dry and prone to cracking.

Home Treatment for Mild Cracked Heels

For mild-to-moderate fissures without deep bleeding cracks, a consistent home routine is effective. After showering, apply a thick urea-based cream (20–40% urea concentration) to the heel and cover with a cotton sock overnight. Urea is a keratolytic — it softens and helps dissolve the thickened callus. Generic 40% urea creams are available at pharmacies. Avoid petroleum jelly (Vaseline) alone — it provides moisture but does not thin the callus. Use a pumice stone or foot file weekly on softened skin, never on dry skin. Stop wearing open-backed shoes until heels have healed.

Professional Treatment for Deep Heel Fissures

Debridement — Dr. Biernacki safely removes the thickened callus tissue surrounding heel fissures using specialized instruments. Removing the callus relieves the mechanical tension pulling the fissures open and allows topical treatments to penetrate effectively. Debridement is painless in patients with intact sensation (no anesthesia needed) and provides immediate relief of pressure.

Prescription heel applications — Prescription-strength urea preparations, hydrocolloid dressings, and cyanoacrylate liquid bandage (medical “super glue”) applied over deep fissures provides waterproofing, pain relief, and helps hold the crack closed while it heals. Studies show cyanoacrylate application provides significantly faster healing of deep heel fissures than topical medications alone.

Addressing underlying causes — If hypothyroidism, fungal infection, or vitamin B/zinc deficiency is contributing to heel cracking, addressing the systemic cause is essential for lasting resolution. Dr. Biernacki coordinates with primary care for lab workup when indicated.

When Cracked Heels Are an Emergency

For patients with diabetes or peripheral vascular disease, cracked heels are a serious medical concern. Deep heel fissures provide a portal of entry for bacteria, and impaired healing means small infections can rapidly become limb-threatening. Any diabetic patient with heel fissures — especially deep ones, or those with associated redness, warmth, or discharge — needs prompt podiatric evaluation. Do not attempt to cut or debride cracked heel skin yourself if you are diabetic.

Frequently Asked Questions

What is the fastest way to heal cracked heels?

The fastest healing combines professional callus debridement (same visit) with daily 40% urea cream application, overnight sock occlusion, and switching to closed-back supportive footwear. Deep fissures can be sealed with medical cyanoacrylate for immediate pain relief. Most patients see significant improvement within 1–2 weeks of consistent treatment.

Can a podiatrist treat cracked heels?

Yes. Podiatrists routinely treat heel fissures, especially severe or infected cases. Professional debridement of the surrounding callus is much more effective than home treatment alone, and is essential for diabetic patients who cannot safely manage their own foot skin. Most insurance plans cover heel fissure treatment when associated with a medical condition.

Do cracked heels indicate a health problem?

Sometimes. Persistent or severe heel cracking can indicate hypothyroidism, diabetes, peripheral vascular disease, eczema, psoriasis, or vitamin deficiency. If over-the-counter moisturizers haven’t helped after 2–4 weeks of consistent use, a podiatric evaluation is appropriate to rule out systemic contributors.
