Quick answer: Pitted Keratolysis Feet is a common foot/ankle topic that affects many patients. The 2026 evidence-based approach combines proper diagnosis, conservative-first treatment, and escalation only when needed. We treat this regularly at our Howell and Bloomfield Township practices. Call (810) 206-1402.
The most important clinical decision with Pitted Keratolysis Feet isn’t which treatment to start with — it’s identifying the correct subtype. That changes everything. Call (810) 206-1402.
What Is Pitted Keratolysis?
Pitted keratolysis is a superficial bacterial infection of the skin on the weight-bearing surfaces of the feet — primarily the heel, ball of foot, and toes. The bacteria (most commonly Corynebacterium, Kytococcus sedentarius, and Dermatophilus congolensis) digest keratin, creating characteristic small pits or craters in the skin. As bacteria multiply, they release sulfur compounds — the source of the notoriously foul smell that often brings patients in for care.
Causes and Risk Factors
Pitted keratolysis thrives in warm, moist environments. Risk factors include:
- Excessive foot sweating (hyperhidrosis)
- Prolonged wearing of occlusive footwear (boots, rubber-soled shoes)
- Athletes, military personnel, outdoor workers
- Not changing socks daily or wearing synthetic non-breathable socks
- Hot, humid climates or seasons
Symptoms
- Multiple small (1–5mm) pits or craters on the sole, heel, or toe pads
- White or grayish maceration of the skin around the pits
- Intense, foul foot odor (often described as a cheesy or sulfurous smell)
- Mild itching or burning; rarely painful unless severe
- Worse after a long day in shoes or in hot weather
Treatment
Topical Antibiotics (First-Line)
Topical erythromycin 2% solution or clindamycin 1% solution applied to the affected areas twice daily is the standard first-line treatment. Apply after thoroughly drying the feet, and allow to dry completely before putting on socks. Continue for 3–4 weeks even after the pits resolve. Benzoyl peroxide 5–10% gel used alongside antibiotics accelerates clearance and reduces recurrence risk.
Antiperspirant for Feet
Aluminum chloride hexahydrate (Drysol or Certain Dri) applied to the soles 2–3× per week dramatically reduces foot sweating — removing the moist environment bacteria need. This is the most important step for preventing recurrence. Apply at night to dry, clean feet; wash off in the morning.
Hygiene Modifications
Hygiene changes accelerate healing and prevent return: change socks daily (preferably moisture-wicking merino wool or copper-infused fabric), allow shoes to dry fully between wearings, alternate between two pairs of shoes, use antifungal powder in shoes (even though this is bacterial, the powder reduces moisture), and wash feet with antibacterial soap daily.
Key takeaway: Pitted keratolysis is almost always curable within 3–4 weeks of consistent topical antibiotic treatment. The key to preventing recurrence is controlling foot moisture — daily sock changes, foot antiperspirant, and breathable footwear.
⚠️ See a podiatrist if:
- No improvement after 4 weeks of topical antibiotics — may need oral antibiotics
- Condition keeps returning within weeks of clearing — need to address hyperhidrosis
- You’re not sure if it’s pitted keratolysis vs. toenail fungus or athlete’s foot (different treatments)
- You’re immunocompromised — infection can be more widespread and harder to clear

Frequently Asked Questions
Is pitted keratolysis contagious?
Pitted keratolysis is not considered highly contagious under normal circumstances, but the causative bacteria can theoretically spread through direct skin contact or shared footwear. Standard precautions — not sharing shoes, socks, or towels — are sensible while being treated. The condition is not a form of athlete’s foot (which is fungal) and is not related to poor personal hygiene per se, though moisture control is key.
What does pitted keratolysis look like?
The hallmark appearance is multiple tiny (1–5mm) crater-like pits on the heel, ball of foot, or toe pads, often with white surrounding maceration. In severe cases, the pits can coalesce into larger eroded patches. The foul odor is often noticed by patients before they notice the pits. It is distinct from callus (thickened uniform skin) and from athlete’s foot (scaly, itchy, between toes).
Can pitted keratolysis go away on its own?
Mild cases occasionally improve with improved foot hygiene and ventilation alone, but most cases persist or worsen without antibiotic treatment. The bacteria are thriving in an optimal environment; simply airing your feet will help but rarely fully clears an established infection. Topical antibiotics are inexpensive, fast, and highly effective — don’t wait to treat it.
The Bottom Line
Pitted keratolysis is a very common but underdiagnosed condition — many patients live with the embarrassing odor and white pits for months assuming it’s athlete’s foot or just “sweaty feet.” A 3–4 week course of topical antibiotics combined with foot antiperspirant clears it reliably. If it keeps coming back, prescription-strength hyperhidrosis treatment provides a long-term solution.
