When Surgery Is Indicated
Plantar fasciitis surgery is a last resort — not an early option. The threshold for surgical candidacy: minimum 12 months of symptoms, completion of all conservative measures (stretching, orthotics, physical therapy), and failure of at least one advanced intervention (cortisone injection, PRP, or extracorporeal shockwave therapy). Fewer than 5% of plantar fasciitis patients ultimately need surgery. Rushing to surgery before completing conservative care produces poorer outcomes and unnecessary risk.
Surgical Options
Endoscopic plantar fascia release (EPFR): Two small portals (5mm incisions) on each side of the heel. Advantages: minimal incision, faster soft tissue healing, return to shoe earlier. Most common approach in my practice for appropriate candidates.
Open plantar fascia release: A 3–4 cm incision over the medial heel allows direct visualization and release of the fascia. May be preferred when heel spur removal is needed simultaneously, or when anatomy is complex. Longer soft tissue healing than endoscopic approach.
Gastrocnemius recession: Often performed simultaneously or as a standalone procedure when severe equinus (Achilles tightness) is the primary driver. Release of the gastrocnemius aponeurosis at the mid-calf level increases ankle dorsiflexion and reduces plantar fascia tension without requiring fascia release. Excellent outcomes for equinus-driven fasciitis.
Recovery Timeline
Week 0–2: Non-weight-bearing in a surgical boot. Wound care and elevation. Ice for swelling. Seated exercises (gentle ankle pumps, toe curls).
Week 2–6: Gradual protected weight-bearing in the boot. Begin physical therapy (range of motion, scar mobilization, progressive strengthening). Pain typically 2–3/10 in the boot by week 4.
Week 6–8: Transition from boot to supportive shoe with orthotic. Normal walking increasing progressively. PT continues with strengthening and gait retraining.
Month 3–4: Return to running and sport for most patients. Full return to unrestricted activity typically at 16–20 weeks.
