Quick Answer
Toenail fungus (onychomycosis) is a dermatophyte infection causing yellow-brown discolouration, thickening, and brittleness. It will not resolve without treatment. OTC options work only for very early cases — established infection needs oral terbinafine or laser therapy.

Toenail Fungus Before & After — Real Laser Treatment Results from Michigan Patients
You’ve tried the creams. You’ve tried the home remedies. You’ve watched the fungus spread from one nail to the next. We get it — we see patients every day who’ve spent years and hundreds of dollars on treatments that didn’t work. Here’s what actually works — and the proof.
Case Study #1: The Runner
A 45-year-old active female came to us with fungal infection on 3 toenails that had persisted for over 4 years. Yellow, thickened nails had kept her from wearing sandals and were affecting her confidence during races. She’d tried OTC lacquers and tea tree oil without success.
Treatment: Toenail fungus laser — 3 sessions over 3 months. Each 20-minute session was painless with no downtime. She walked out of every appointment without restrictions.
Results: Significant clearing at 6 months. Clear, healthy nails at 12 months. She’s back in sandals and running 5Ks.
Maintenance products she uses: Antifungal shoe spray (daily), UV shoe sanitizer, moisture-wicking socks, tea tree nail treatment between visits.
Case Study #2: The Diabetic Patient
A 68-year-old male with Type 2 diabetes presented with fungal infection on all 10 toenails. The nails were severely thickened and dystrophic — impossible to trim safely at home, and a serious infection risk for a diabetic patient.
Treatment: Laser therapy coordinated with care from Vassallo Medical Group in Howell for blood sugar management. Proper glucose control accelerates nail regrowth.
Results: Gradual clearing as new healthy nail grew in. Regular medical nail care visits (covered by Medicare for at-risk diabetics) maintained his results. His wife says he finally lets her see his feet again.
Our Howell office coordinates with Vassallo Medical Group (same road — Grand River Ave) for patients with diabetes, vascular disease, and systemic conditions. Coordinated care for Livingston County patients.

Case Study #3: The Worst Case That Still Worked
The most advanced case of toenail fungus we’ve successfully treated involved years of ignored infection, severely deformed nails, and previous failed oral medication. Even this patient achieved significant improvement. If we can fix the most severe cases — imagine what we can do for you at an earlier stage.
How Our Laser Treatment Works
Our toenail fungus laser is a painless, 20-minute in-office procedure with zero downtime. Here’s why it works when creams don’t:
- Laser energy penetrates the nail plate to reach the fungus living beneath — where topical treatments can’t reach
- Heat destroys the fungal cells without harming surrounding tissue
- No oral medication means no liver monitoring required
- Typically 3–4 sessions spaced 4–6 weeks apart
- New, healthy nail grows in over the following 6–12 months
The Maintenance Kit — Keep Your Results

Insurance & Cost
Toenail fungus laser treatment is typically not covered by insurance — however, your evaluation visit IS covered under most plans. We verify your benefits before every appointment. We offer payment plans and accept HSA/FSA cards for laser treatment.
Frequently Asked Questions
Does laser treatment hurt? No — patients feel mild warmth during the procedure. It’s comfortable enough that most patients read their phone during treatment.
How long until I see results? The fungus is eliminated at the nail root, but you’ll see new healthy nail growing in over 6–12 months as it replaces the old nail.
Will the fungus come back? Not if you follow our maintenance protocol — especially treating your shoes with antifungal spray daily.
Can I paint my nails during treatment? We recommend keeping nails clear of polish during the treatment phase so we can monitor progress accurately.
How much does it cost? We provide transparent pricing at your consultation. We offer payment plans and accept HSA/FSA.

Differential Diagnosis: What Else Could It Be?
Several conditions share symptoms with Toenail Fungus and are commonly misdiagnosed in the first office visit. Considering these alternatives is part of every Balance Foot & Ankle exam:
- Nail psoriasis. Pitting, oil-drop spots, salmon patches — responds to topical steroid, not antifungal.
- Nail trauma (runner’s nail). Subungual hematoma from repetitive impact — shoe fit fix, not antifungal.
- Onychogryphosis. Thickened, ram’s-horn nail of the elderly — debridement, not pills.
If your symptoms don’t fit the textbook pattern, ask your podiatrist which differentials they ruled out — that conversation often shortcuts months of trial-and-error treatment.
In Our Clinic
In our clinic, the toenail fungus patient has usually already tried OTC topical antifungals for months with no improvement. We confirm the diagnosis with a PAS stain or fungal culture — because about 30 % of thickened, discolored nails are actually NOT fungal (they’re trauma, nail psoriasis, or keratin granulation from polish). For true dermatophyte onychomycosis, oral terbinafine (Lamisil) remains the most effective treatment with ~70 % cure rate and manageable safety monitoring. Nail laser and topical efinaconazole are options for patients who can’t take oral medication. The nail grows back clear over 9–12 months, not overnight.
Most Common Mistake We See
The most common mistake we see is: Applying OTC antifungal cream onto a lifted or thickened nail where it can’t penetrate. Fix: oral terbinafine or laser therapy for anything beyond early-stage surface discolouration.
