When to Go to the ER for Foot Pain: A Practical Guide

Not every foot problem warrants an emergency room visit, and many do not. But some foot injuries and conditions represent genuine emergencies where delayed treatment causes significantly worse outcomes. Knowing which category your situation falls into saves you from both unnecessary ER visits and dangerous delays in care.

Go to the Emergency Room Immediately For:

Open fractures — where bone is visible through the skin or a wound is directly over a fracture site — require emergency care for wound management, fracture stabilization, and infection prevention. Foot or ankle injuries with absent pulses, severe swelling that is rapidly worsening, extreme pallor or blueness of the foot, or severe pain out of proportion to the apparent injury may indicate a vascular injury or compartment syndrome — a surgical emergency where elevated pressure in the fascial compartments destroys muscle and nerve tissue within hours. Signs of serious foot infection including a rapidly spreading red line tracking up the leg (lymphangitis), high fever combined with foot pain and swelling, foul-smelling discharge, black or purple skin discoloration around a wound, or signs of sepsis (confusion, racing heart, very low blood pressure) require emergency evaluation. Deep puncture wounds — particularly from nails, glass, or organic material — carry high infection risk and may require irrigation, debridement, and prophylactic antibiotics beyond what urgent care can provide. Complete Achilles tendon rupture — the classic sudden snap during activity followed by inability to push off — benefits from prompt evaluation to determine the treatment approach, though it is not immediately life-threatening. Dislocations of the ankle or toe joints visible as obvious deformity require reduction, ideally within hours.

Urgent Care or Next-Day Podiatry Visit (Not ER) For:

Most ankle sprains — unless deformity is present or weight-bearing is completely impossible — are appropriately evaluated at urgent care or by your podiatrist within 1 to 2 days. Simple metatarsal fractures without deformity that allow some weight-bearing can be assessed at urgent care or a podiatry office rather than the ER. Any new wound that is not healing within 24 to 48 hours, any wound showing signs of infection, any rapid swelling of a single foot (which may represent Charcot neuroarthropathy), or any foot discoloration in a diabetic patient warrants same-day evaluation — at minimum urgent care or podiatry, and the ER if those options are not available. The diabetic foot can deteriorate from a minor-appearing problem to a limb-threatening infection within 24 to 48 hours, particularly in patients with neuropathy who do not feel pain as a warning signal.
