Best hammertoe splints 2026: The BioSkin Hammer Toe Straightener is our #1 podiatrist pick for structural correction during activity. ZenToes Gel Correctors are best for all-day cushioning and mild deformity. No splint corrects a fixed hammertoe — but in flexible hammertoes, consistent splinting combined with appropriate footwear can slow progression and reduce pain significantly.

Understanding Hammertoe: What Splints Can and Can’t Do

Hammertoe is a progressive flexion deformity of the proximal interphalangeal (PIP) joint — the middle joint of the toe bends downward, creating the characteristic hammer shape. The second, third, and fourth toes are most commonly affected. It develops from a combination of intrinsic muscle weakness, footwear-induced toe compression, and — in many cases — an underlying bunion that pushes the second toe out of alignment.

What splints can do: In flexible hammertoe (early stage, toe can be manually straightened), consistent splinting reduces pain, prevents further contracture, and improves toe alignment during walking. This is genuinely meaningful conservative management.

What splints cannot do: A rigid hammertoe (late stage, joint is fixed in flexion) cannot be corrected with splinting — surgical arthroplasty or arthrodesis is required. If your toe doesn’t straighten when you pull it gently by hand, you have a rigid hammertoe and should schedule a podiatric evaluation.

⚠️ Signs Your Hammertoe Needs Professional Evaluation:
• The toe is rigid and cannot be manually straightened
• You have a corn or ulcer developing on top of the bent joint
• The toe is crossing over or under an adjacent toe
• You have diabetes or circulation problems affecting your feet
• Significant pain with weight bearing or shoe wear despite splinting

Best Hammertoe Splints & Correctors 2026

🥇 #1 Best Structural Correction: BioSkin Hammer Toe Straightener

The BioSkin Hammer Toe Straightener uses a patented strap-and-wrap design that applies a consistent three-point pressure system to gently extend the PIP joint. Unlike gel-only devices that primarily cushion, the BioSkin provides active mechanical correction — pulling the toe toward neutral alignment and holding it there during activity. Our first recommendation for patients with flexible hammertoe who need functional correction during daily walking. The compression foot wrap stabilizes the entire forefoot, and the adjustable toe straps accommodate different deformity severities. Works for hammertoe, mallet toe, dorsal drift, and floating toe presentations.

🥈 #2 Best Daily Cushioning: ZenToes Triple-Loop Gel Corrector

The ZenToes gel corrector takes a different approach: instead of active mechanical correction, it provides a soft gel cushion under the proximal phalanx that lifts the toe from below while three stretchy loops maintain alignment. This design excels at pain relief — particularly for patients whose hammertoe creates painful corn formation on the dorsal (top) surface of the bent joint from shoe friction. The gel material distributes pressure away from the corn and reduces the grinding of the bent joint against shoe leather. Comfortable for all-day wear inside most shoes. 4-pack provides excellent value.

#3 Best for Broken/Injured Toes: Dimeho Fabric Toe Splints — 8 Pack

The Dimeho fabric toe splints use buddy-taping principles — wrapping the affected toe to an adjacent healthy toe to maintain alignment. While most commonly used for broken toes, this approach is also effective for mild hammertoe management and post-procedure protection after hammertoe surgery. The 8-pack provides excellent value for daily replacement, important for maintaining hygiene during extended use. The reusable fabric construction washes easily. Our recommendation for post-surgical hammertoe patients transitioning out of surgical footwear.

#4 Best Precision Correction: Pozpo Tech Aluminum-Core Hammertoe Splint

The Pozpo Tech splint incorporates a precision-machined aluminum plate into the splint base, applying a rigid three-point force system directly to the bent PIP joint. This is the most mechanically direct correction device in our review — it physically prevents the toe from flexing into its deformed position during the gait cycle. Best for patients with moderate hammertoe who have tried soft gel devices without satisfactory structural improvement. The aluminum core makes this inappropriate for patients with metal sensitivities.

#5 Best Toe Spacer Combination: Gel Toe Separator & Spreader

Many hammertoe patients also have adjacent toe crowding — particularly when a bunion has pushed the big toe into the second toe’s space. A toe separator worn alongside a hammertoe splint provides dual benefit: correcting the hammertoe flexion deformity while also preventing the compressive forces that accelerate it. Our recommendation for patients with concurrent bunion and hammertoe presentations.

Hammertoe Treatment: The Full Conservative Protocol

Splinting is one component of a conservative hammertoe management plan. The complete approach includes: wide toe box footwear that allows the toes to splay without compression; hammertoe splint worn inside the shoe during activity; custom orthotics when biomechanical driving factors (flatfoot, bunion, high arch) are contributing; intrinsic foot muscle strengthening exercises (toe curls, marble picking, towel scrunches) to restore balance between flexor and extensor muscle groups; and corn management with a podiatrist when painful corns develop over the bent joint.

When to See a Podiatrist

Rigid hammertoes don’t reduce with splinting alone — the tendon and capsule have contracted. If the toe no longer straightens passively, surgical correction restores alignment in one short outpatient visit. Call Balance Foot & Ankle to see whether your deformity is still flexible (and responsive to the conservative tools above) or if it’s time for a 20-minute in-office correction.

FAQ

Can hammertoe splints reverse a hammertoe?

In early-stage flexible hammertoe, consistent splinting combined with appropriate footwear can prevent progression and sometimes improve alignment meaningfully over months. True reversal of an established deformity is not achievable with conservative measures — but halting progression and achieving comfortable pain-free function is a realistic and worthwhile goal for most patients.

How long should I wear a hammertoe splint daily?

For flexible hammertoe, we recommend wearing a splint during all weight-bearing activity — inside supportive shoes during walking, standing, and exercise. Many patients benefit from wearing a softer gel corrector at night as well. Consistency over months produces the best outcomes for slowing deformity progression.

Differential Diagnosis: What Else Could It Be?

Several conditions share symptoms with Hammertoe and are commonly misdiagnosed in the first office visit. Considering these alternatives is part of every Balance Foot & Ankle exam:

  • Mallet toe. Bend at the distal joint only (DIP), not the middle joint.
  • Claw toe. Both joints (PIP + DIP) bent — usually multiple toes, often diabetic.
  • Crossover toe (2nd MTP). Drift across the big toe — often plantar plate tear underneath.

If your symptoms don’t fit the textbook pattern, ask your podiatrist which differentials they ruled out — that conversation often shortcuts months of trial-and-error treatment.

In Our Clinic

Hammertoes come to our clinic in two flavors: flexible (the toe still passively straightens) and rigid (it doesn’t). For flexible hammertoes we use gel toe crests, roomier toe boxes, custom orthotics to address the underlying instability, and sometimes night splints. Rigid hammertoes with a corn on top of the PIP joint, or a callus under the metatarsal head, usually need a minor outpatient procedure (PIP arthroplasty or fusion) to straighten the toe. The patients who wait too long develop fixed deformities and skin breakdown — we would much rather address a flexible hammertoe early.

Most Common Mistake We See

The most common mistake we see is: Cutting corns at home with scissors or razors. Fix: professional podiatric enucleation with sterile instruments prevents infection and recurrence.

Warning Signs That Need Same-Day Care

Seek immediate evaluation at Balance Foot & Ankle if you experience any of the following:

  • Open wound under the bent toe
  • Inability to straighten the toe even passively
  • Diabetic skin breakdown at the toe joint
  • Cold or blue discolouration of the toe
