Deep vein thrombosis (DVT) and pulmonary embolism (PE) are recognized complications of foot and ankle surgery, with estimates of DVT incidence ranging from 5–36% depending on detection method (clinical vs. routine duplex ultrasound), procedure type, and chemoprophylaxis use. Understanding risk stratification, available prophylaxis options, and the clinical signs of DVT allows patients and clinicians to manage this risk intelligently without either overreating low-risk patients or undertreating high-risk ones.

Why Foot and Ankle Surgery Carries DVT Risk

Virchow’s triad — venous stasis, hypercoagulability, and endothelial injury — applies directly to foot and ankle surgery. Post-operative immobilization in a cast or boot reduces calf muscle pump activity, producing venous stasis in the deep calf veins. Surgical trauma activates the coagulation cascade. Lower extremity surgery produces endothelial injury. Non-weight-bearing restrictions amplify all three risk factors by eliminating the dynamic calf pump activation that occurs with normal walking.

Risk Stratification

Risk factors that significantly elevate DVT risk include prior VTE history, age over 60, BMI over 35, active malignancy, thrombophilia, oral contraceptive or hormone therapy use, prolonged non-weight-bearing (more than 4 weeks), and total ankle replacement or hindfoot/rearfoot fusion procedures (which involve more extensive tissue disruption and longer immobilization than forefoot procedures). Forefoot procedures (bunion correction, hammertoe correction) in young, healthy patients with normal ambulation potential carry low DVT risk and typically do not require pharmacological prophylaxis.

Prophylaxis Options

Low-molecular-weight heparin (LMWH — enoxaparin, dalteparin) is the most evidence-supported pharmacological prophylaxis for high-risk foot and ankle surgery patients, typically administered for 2–4 weeks post-operatively. Aspirin 81mg daily has evidence supporting its use in lower-risk patients and is preferred by some surgeons for its simplicity and low cost. Mechanical prophylaxis — compression stockings on the contralateral limb during surgery and early rehabilitation — is appropriate for all patients regardless of pharmacological decisions. Intermittent pneumatic compression devices are effective but difficult to apply to the post-operative surgical extremity.

Recognizing DVT Post-Operatively

The Wells Score criteria — calf swelling greater than 3cm compared to the contralateral limb, pitting edema confined to the symptomatic limb, recently bedridden, calf tenderness along deep vein distribution — apply but overlap with normal post-operative findings. Unexplained calf swelling disproportionate to the surgical limb, new pain in the thigh or calf without clear surgical explanation, or shortness of breath or chest pain warrants duplex ultrasound or CT pulmonary angiography respectively.
