Steroid-Induced Foot Problems — When Prednisone Hurts Your Feet
Long-Term Steroids and Your Feet: What You Need to Know
Corticosteroids (prednisone, methylprednisolone, dexamethasone) are among the most widely prescribed medications in medicine — used for asthma, COPD, inflammatory bowel disease, rheumatologic conditions, organ transplant rejection prevention, and many other conditions. Long-term steroid use creates a specific set of foot problems that Balance Foot & Ankle manages regularly for patients across Michigan.
Steroid-Induced Osteoporosis and Stress Fractures
Corticosteroids inhibit bone formation and increase bone resorption — accelerating bone density loss throughout the skeleton, including the feet. Patients on long-term steroids are at significantly elevated risk for metatarsal stress fractures, ankle insufficiency fractures, and Charcot-like bone collapse in the midfoot. EMTT magnetotransduction is particularly valuable for accelerating bone healing in steroid-treated patients with stress fractures.
Tendon Weakness and Rupture
Steroids weaken tendon collagen, increasing the risk of Achilles tendon and posterior tibial tendon rupture. Patients on steroids who develop sudden calf or arch pain warrant urgent evaluation.
Impaired Wound Healing
Steroids suppress the inflammatory response that drives wound healing — even minor skin breaks can heal slowly and become infected. Steroid-treated patients need the same careful, protective approach to foot care as diabetic or immunocompromised patients.
Steroid-Induced Diabetes
Long-term steroids can cause or worsen hyperglycemia (steroid-induced diabetes), adding diabetic foot risk to the existing steroid-specific risks. We coordinate with endocrinology and primary care for these patients.
