When is night pain an emergency?

If pain forces you to hang the foot off the bed, takes standing up to relieve, or is accompanied by cold, pale, or blue-tinged feet — this is “rest pain” and can indicate critical limb ischemia (severe peripheral arterial disease). Go to the ER or vascular surgeon. Also urgent: night pain with fever, rapidly worsening pain, or swelling.

Is burning foot pain at night neuropathy?

Often yes. Small-fiber diabetic neuropathy classically peaks at night when you're still. Causes: diabetes, B12 deficiency, alcohol, chemotherapy, hypothyroidism, or idiopathic. First-line treatment: gabapentin or duloxetine, B12 supplementation if deficient, glucose optimization. Topical capsaicin or lidocaine patches as adjuncts. Most patients improve significantly with a multi-modal approach.

Why does plantar fasciitis hurt at night?

Most plantar fasciitis hurts most in the morning (first-step pain), but 20-30% of patients also get night pain or pain that wakes them. The mechanism: while asleep, the fascia shortens; any middle-of-night bathroom trip stretches it forcefully. Solution: night splint maintaining dorsiflexion. Gel heel cups for bed-exit walking. Consistent daytime stretching.

What about restless legs syndrome?

Classic pattern: creeping/crawling sensation in the legs/feet in the evening, irresistible urge to move, relieved by movement. Distinct from neuropathy (which doesn't improve with movement). First-line: check iron (ferritin under 75 = supplement), avoid caffeine after noon, regular exercise. Medications like pramipexole or gabapentin work for persistent cases. Not usually a podiatry problem — refer to neurology or sleep medicine.
