A sprained little (fifth) toe is a stretch or partial tear of the ligaments around the toe joints, usually from stubbing it, a fall, or jamming it into furniture. It is one of the most common minor foot injuries we see, and the good news is that most heal well at home. The challenge is telling a sprain from a break — and knowing the few situations that need a podiatrist.
Sprained vs. broken little toe
A sprain injures the ligaments; a break (fracture) cracks the bone. Both cause pain, swelling, and bruising, so you often cannot tell them apart by symptoms alone. Signs that point more toward a fracture: a toe that looks crooked or deformed, pain directly on the bone, a snapping sensation at the time of injury, inability to walk, or pain that is still severe after a few days. When in doubt, an X-ray settles it — and a broken toe is still usually treated with buddy taping unless it is badly displaced.
How bad is it? Sprain grades
- Grade 1 (mild): ligaments stretched. Mild swelling and tenderness; you can still walk. Heals in about 1–2 weeks.
- Grade 2 (moderate): partial ligament tear. More swelling, bruising, and pain with movement. Heals in about 3–6 weeks.
- Grade 3 (severe): complete tear or dislocation. Significant swelling, instability, and difficulty bearing weight. Needs evaluation and takes longer.
Buddy taping: step by step
Buddy taping splints the injured toe against its healthy neighbor so it can rest and heal in a stable position.
- Place a small piece of padding (gauze or a gel toe spacer) between the two toes to prevent skin rubbing and blisters.
- Gently tape the little toe to the fourth toe using a soft, self-adherent wrap — snug, not tight enough to cut off circulation.
- Re-check that the toes are pink and not numb. Loosen if they tingle or change color.
- Replace the tape daily and keep the skin dry. Continue for 2–4 weeks depending on the grade.

Home care (RICE) and footwear
Use RICE — Rest, Ice (15–20 minutes with a cloth barrier), Compression (the buddy wrap), and Elevation. A stiff-soled shoe or a roomy, open shoe limits painful bending of the toe; avoid tight or pointed shoes until the swelling settles. Over-the-counter anti-inflammatories can help with pain if appropriate for you.
When to get an X-ray or see a podiatrist
- The toe looks crooked, rotated, or deformed
- You cannot bear weight or walk
- Severe pain, numbness, or the toe is cold or pale
- An open wound or the nail is involved
- No improvement after about a week of buddy taping
- You have diabetes or poor circulation — see a podiatrist for any toe injury

How long does a sprained little toe take to heal? Mild sprains settle in 1–2 weeks; moderate ones in 3–6 weeks. Bruising can linger after the pain is gone.
Should I walk on a sprained toe? Light walking is usually fine if you can tolerate it; let pain be your guide and wear a stiff-soled shoe. If you cannot bear weight, get it checked.
