To qualify, the patient must: have diabetes mellitus (any type); have one of the following: peripheral neuropathy with evidence of callus formation, a history of pre-ulcerative calluses, a history of previous ulceration, foot deformity, poor circulation, or prior partial or complete foot amputation; and receive a written prescription from their treating physician or podiatrist.

A certified pedorthist or shoe fitter then measures the foot and selects appropriate shoe models. Custom insoles are fabricated from a foot impression and delivered to the patient’s selected shoes.
