They present with acute plantar first metatarsal head pain that mimics the chronic sesamoiditis seen in dancers and runners, but require a distinctly different acute management approach. The principal diagnostic challenge is distinguishing an acute fracture from the bipartite sesamoid — a normal developmental variant present in 10–30% of the population — that can appear identical on plain X-ray.

Diagnosis and Acute Treatment
Clinical features: acute onset plantar first MTP pain following a specific traumatic event; significant tenderness with direct palpation of the sesamoid through the plantar surface; pain with passive dorsiflexion of the hallux (which loads the sesamoids maximally). X-ray: axial sesamoid view (the ‘sesamoid view’) — identifies a fracture line; bipartite sesamoid has smooth, rounded cortical margins with a wider gap than an acute fracture; comparison views of the contralateral foot are helpful. They describe pain directly UNDER the big toe joint β not at the joint (that’s hallux rigidus) β which worsens with push-off. On exam we palpate each sesamoid separately (tibial and fibular) and assess for sensitivity. We always get X-rays to look for sesamoid fracture or bipartite sesamoid (a normal variant). Treatment uses a dancer’s pad to offload the sesamoid, stiff-soled footwear to reduce push-off stress, and activity modification.
