Magnetic resonance imaging (MRI) is the gold-standard diagnostic tool for soft tissue pathology, bone marrow edema, and osteochondral lesions — conditions that are completely invisible on plain X-ray. Dr. Tom Biernacki’s diagnostic algorithm uses MRI selectively: when clinical and ultrasound findings suggest pathology that requires soft tissue characterization or when bone stress injury is suspected but X-rays are negative.
Conditions Requiring MRI Diagnosis
Stress fractures of the foot and ankle represent the most time-sensitive MRI indication. Plain X-rays are negative in the first 2–3 weeks of a stress fracture — the fracture line is not visible until periosteal reaction develops. MRI shows bone marrow edema (the stress reaction) within days of onset, allowing early diagnosis and appropriate management before complete fracture occurs. Navicular, fifth metatarsal, and sesamoid stress fractures are particularly important to catch early due to their high complication risk.
Osteochondral lesions of the talus (OLT) are cartilage and subchondral bone injuries on the ankle joint surface, typically caused by ankle sprains. They are frequently missed on X-ray and cause chronic ankle pain that is misattributed to ligament injury. MRI grades OLT severity (stable vs. unstable, lesion size) and directly guides treatment — stable small lesions are managed conservatively; unstable or large lesions require arthroscopic surgery.
Posterior tibial tendon tears and peroneal tendon tears require MRI for complete characterization. Ultrasound identifies partial and complete tears, but MRI provides superior delineation of tear extent, tendon quality, and the status of the spring ligament complex — all critical for surgical planning in progressive flatfoot or ankle instability.
Plantar fascia tears — partial or complete — occur in approximately 10% of patients treated with corticosteroid injection for plantar fasciitis and are a cause of treatment failure. MRI confirms fascial integrity and guides modified treatment protocols.
Complex ankle instability with multi-ligament involvement, syndesmotic injury, or deltoid ligament disruption requires MRI to define the full injury pattern before reconstructive surgery planning.
Soft tissue masses including ganglion cysts, lipomas, and neoplastic lesions require MRI characterization for surgical planning and to rule out malignancy.
MRI vs. Ultrasound for Foot and Ankle
Ultrasound and MRI are complementary, not competitive, in podiatric imaging. Ultrasound excels at real-time dynamic assessment — evaluating tendon subluxation during movement, compressing a suspected neuroma to reproduce symptoms, and guiding needle placement for injection therapy.
